How safe is it?
Well-documented data
Grape seed extract is generally well tolerated in studies. The most common side effects reported are abdominal pain, diarrhea, dry mouth, indigestion, headache, joint pain, and nausea.
Anaphylaxis to grape skin, though rare, has been reported. Trans-resveratrol is also generally well tolerated at typical supplement doses, though higher doses may cause diarrhea and gastrointestinal discomfort; long-term safety at high doses is not fully established.
For pregnancy: concentrated grape supplements and wine should be avoided due to limited safety data. For breastfeeding: there is not enough information on concentrated grape supplements, and concentrated resveratrol supplements should be avoided.
Grapes as whole food are fine in both situations.

Grape seed extract can affect how your liver breaks down several drug types: it may lower levels of CYP1A2 substrates (theoretically reducing their effect), raise levels of CYP2D6 and CYP2E1 substrates, and either raise or lower CYP3A4 substrates — the evidence is conflicting. It also carries a documented risk with cyclosporine, an immunosuppressant: grape juice can cut cyclosporine absorption by up to 30%, so the two should be separated by at least two hours.
Long-term grape seed use may also reduce the effect of midazolam, a sedative.
Trans-resveratrol poses similar theoretical risks with CYP1A2, CYP2C19, CYP2E1, and CYP3A4 substrates — it may raise their levels, though a human trial of high-dose resveratrol did not require CYP3A4 medication adjustments. We could not check red wine fruit extract against medications — no data is on file for it.

Resveratrol Complex
Anticoagulant/Antiplatelet Drugs
Resveratrol may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Resveratrol seems to have antiplatelet effects.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, resveratrol might increase levels of drugs metabolized by CYP1A2.
In vitro research shows that resveratrol can inhibit CYP1A2 enzymes. However, this interaction has not been reported in humans.
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, resveratrol might increase levels of drugs metabolized by CYP2C19.
In vitro research shows that resveratrol can inhibit CYP2C19 enzymes. However, this interaction has not been reported in humans.
Cytochrome P450 2E1 (Cyp2E1) Substrates
Resveratrol might increase levels of drugs metabolized by CYP2E1.
In vitro research suggests that resveratrol inhibits CYP2E1 isoenzyme. Also, a pharmacokinetic study shows that taking resveratrol 500 mg daily for 10 days prior to taking a single dose of chlorzoxazone 250 mg increases the maximum concentration of chlorzoxazone by about 54%, the area under the curve of chlorzoxazone by about 72%, and the half-life of chlorzoxazone by about 35%. Chlorzoxazone is used as a probe drug for CYP2E1.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, resveratrol might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.
