Not established
The evidence on shiitake mushroom's benefits is thin. For dental plaque, it's rated possibly ineffective.

How safe is it?

Shiitake as a cooked food is generally well tolerated. Concentrated supplements, though, carry more caution because safety data at supplement doses isn't well established.
The most common side effects from shiitake are abdominal discomfort, bloating, diarrhea, nausea, and vomiting. Raw shiitake can cause shiitake dermatitis — a distinctive skin rash with whiplash-like marks — and in rare cases, large inadequately chewed pieces may cause intestinal blockage.
People who handle shiitake extensively (growers, workers) can develop allergic reactions from contact with the mushroom filaments or from inhaling spores, including asthma, rhinitis, and in serious cases, a lung inflammation called hypersensitivity pneumonitis.

Shiitake Mushroom
Cytochrome P450 2D6 (Cyp2D6) Substrates
Theoretically, shiitake mushroom might decrease levels of drugs metabolized by CYP2D6.
In vitro studies suggest that the shiitake mushroom extract AHCC might induce the CYP2D6 enzyme. This effect has not been reported in humans.
Immunosuppressants
Theoretically, taking shiitake mushroom might decrease the effects of immunosuppressive therapy.
In vitro evidence suggests that shiitake mushroom extracts stimulate immune function.
