What’s inside
Low disclosure
Stm contains 16 active ingredients, mostly digestive enzymes. The main ones are amylase, protease, and lipase—all break down carbohydrates, proteins, and fats, respectively.
You also get folate, vitamin B12, and several other enzymes (cellulase, glucoamylase, hemicellulase, diastase, alpha-galactosidase, beta-galactosidase, and phytase) that work on plant material and other complex carbs. The formula rounds out with slippery elm (an herbal demulcent), papaya, mastic tree, and marshmallow—all traditionally used for digestive comfort.
The capsule itself and the inactive ingredients (cellulose and water) are fillers and binders.

The evidence for most of Stm's ingredients is limited. Vitamin B12 is effective for B12 deficiency itself and likely effective for cyanide poisoning; it's also possibly effective for canker sores and nerve pain after shingles.
Mastic tree is possibly effective for indigestion. For the other ingredients—protease, lipase, slippery elm, papaya, marshmallow, and the various other enzymes—we don't have reliable evidence established in our data that they work for any particular condition.
That doesn't mean they don't help; it means the research either isn't there or is too weak to call it.
How safe is it?
Well-documented data
Protease, lipase, and the other digestive enzymes are generally well tolerated, though digestive upset or allergic reactions are possible. Protease can rarely cause allergic reactions from occupational airborne exposure; lipase is generally safe but hasn't been thoroughly studied for side effects.
Slippery elm, papaya, mastic, and marshmallow are generally well tolerated for short-term use, but high-quality safety data are limited for all of them. Papaya can cause nausea, vomiting, or allergic reactions (especially in people allergic to latex); unripe papaya and papaya seeds should be avoided.
Vitamin B12 is very safe with no established upper limit and is well tolerated orally; rarely, allergic rash has been reported, and theoretically severe low potassium (hypokalemia) has been linked to B12 correction of certain anemias.

Before taking this product, check with your pharmacist if you take warfarin or other blood thinners, antidiabetes drugs, amiodarone (a heart rhythm medication), levothyroxine (thyroid hormone), lithium (a mood stabilizer), or metformin. Slippery elm and marshmallow may also slow absorption of oral medications in general—spacing them 30–60 minutes apart may help.

The most serious concern is papaya, which has Moderate interactions with antidiabetes drugs (including glibenclamide), amiodarone, warfarin, and levothyroxine. Fermented papaya preparations may add to the blood-sugar-lowering effect of diabetes medications; papaya extract may raise amiodarone levels; papaya containing papain may increase warfarin's blood-thinning effects; and eating large quantities of papaya fruit may reduce how well your body uses levothyroxine.

Slippery elm carries a Moderate interaction with oral drugs generally—its mucilage can slow how your digestive system absorbs them. Marshmallow has Moderate and Minor interactions: it may theoretically increase lithium levels (a concern for people on this mood stabilizer), and its mucilage may interfere with absorption of other oral medications (spacing doses by 30–60 minutes may help).
Marshmallow flower may also have minor antiplatelet effects, though the root and leaf are more common in supplements.
Vitamin B12 has a Minor interaction with metformin, which can lower your B12 levels with long-term use. Marshmallow
Lithium
Theoretically, due to potential diuretic effects, marshmallow might reduce excretion and increase levels of lithium.
Marshmallow is thought to have diuretic properties. To avoid lithium toxicity, the dose of lithium might need to be decreased when used with marshmallow.
Anticoagulant/Antiplatelet Drugs
Theoretically, marshmallow flower might have antiplatelet effects.
Animal research suggests that marshmallow flower extract has antiplatelet effects. However, the root and leaf of marshmallow, not the flower, are the plant parts most commonly found in dietary supplements. Theoretically, use of marshmallow flower with anticoagulant/antiplatelet drugs can have additive effects, and might increase the risk for bleeding in some patients.
Oral Drugs
Theoretically, mucilage in marshmallow might impair absorption of oral drugs.
Marshmallow contains mucilage which can affect oral drug absorption. To avoid changes in absorption, take marshmallow 30-60 minutes after oral medications.
Slippery Elm
Oral Drugs
Theoretically, slippery elm may slow the absorption and reduce serum levels of oral drugs.
Slippery elm inner bark contains mucilage, which may interfere with the absorption of orally administered drugs.
Papaya
Amiodarone (Cordarone)
Theoretically, papaya extract may increase the levels and clinical effects of amiodarone.
Animal research in rats shows that a single oral dose of papaya extract, as well as multiple doses of papaya extract daily over 14 days, prior to a single dose of amiodarone delays the time to maximum amiodarone concentration. However, only the 14-day papaya extract regimen increases systemic amiodarone exposure by 60% to 70%. This interaction has not been reported in humans.
Antidiabetes Drugs
Concomitant use of antidiabetic drugs with fermented papaya can produce additive effects. It is unclear if other forms of papaya have the same effect.
A small low-quality clinical study in patients with type 2 diabetes who are taking glibenclamide shows that taking a fermented papaya preparation 3 grams daily for 2 months decreases fasting and postprandial blood glucose levels when compared to baseline. Additionally, of the 25 patients in the study, 9 required a reduction in glibenclamide dose.
Levothyroxine (Synthroid, Others)
Theoretically, consuming large quantities of papaya fruit can reduce the clinical effects of levothyroxine.
In one case-report, a 37-year-old male with a history of thyroidectomy who was stabilized on levothyroxine for 5 years presented with hypothyroidism after consuming 5-6 papaya fruits daily for 14 days during vacation. In a controlled re-challenge test involving 5-6 papayas daily, the patient remained euthyroid for 7 days, but developed mild hypothyroidism after 14 days. Both times, thyroid levels normalized 40-45 days after discontinuing papaya.
Warfarin (Coumadin)
Theoretically, concomitant use of warfarin with papain-containing papaya extract might increase the effects and side effects of warfarin.
In one case report, a patient previously stable on warfarin was found to have an international normalization ratio (INR) of 7.4, which was attributed to ingestion of a supplement containing papain from papaya extract.
Vitamin B12
Metformin (Glucophage)
Metformin, a common medication used to manage type 2 diabetes, has been associated with lower vitamin B12 levels in some individuals. Prolonged use of metformin can interfere with the absorption of B12 in the digestive system, potentially leading to a deficiency in this essential vitamin.
