What is this page for?

What’s inside
Full disclosure
Super Cleans-R contains seven active ingredients. Marshmallow, amalaki (Indian gooseberry), clove, cascara sagrada, black walnut, wormwood, and senna are all included to support the product's cleansing purpose.
One inactive ingredient, cellulose, serves as a filler.
Does it work?
Strong evidence
The evidence supporting these ingredients is limited. Marshmallow has insufficient reliable evidence for the conditions it's traditionally used for, including vaginal candidiasis, diarrhea, and respiratory infections.
Indian gooseberry is possibly effective for gastroesophageal reflux disease (GERD) and possibly effective for high cholesterol, but insufficient evidence exists for other uses. Clove is possibly effective for ventilator-associated pneumonia when used medically but has insufficient evidence for cough, acute pain, and indigestion.
Cascara sagrada has insufficient evidence for constipation and appears possibly ineffective for bowel preparation. Black walnut and wormwood lack sufficient evidence for their traditional uses.
Senna is likely effective for constipation and possibly effective for bowel preparation, but it's not meant for long-term use.
How safe is it?
Well-documented data
Most of these ingredients are generally well tolerated in the short term, but several carry real cautions. Marshmallow and black walnut have limited human safety data; avoid marshmallow during pregnancy and breastfeeding due to insufficient information.
Amalaki is generally well tolerated as a food but supplement safety is less studied—avoid during pregnancy. Clove is safe as a spice but medicinal doses are not well studied during pregnancy or breastfeeding; concentrated clove oil can be toxic, especially in children, and has caused serious effects including liver failure and seizures in rare cases.
Cascara sagrada and senna are stimulant laxatives not meant for pregnancy or breastfeeding and should be used short-term only; long-term use of cascara can cause muscle wasting and blood in the urine. Wormwood contains thujone, a neurotoxin; avoid during pregnancy as it may stimulate the uterus, and avoid breastfeeding due to insufficient safety data.
Trial data for individual ingredients are sparse—amalaki produced upper stomach discomfort or indigestion in a small number of participants, and one participant reported fatigue.
Meds to double-check
Moderate interaction found
Before taking Super Cleans-R, check your medications against these drug types: anticonvulsants (seizure drugs), blood thinners and antiplatelet agents like aspirin and clopidogrel, diabetes medications, diuretics, corticosteroids, digoxin, warfarin, estrogen-based birth control, and any drug metabolized by CYP2D6, CYP1A2, CYP3A4, or CYP2C9 liver enzymes. Use the medication checker on this page with your exact prescriptions.
The bottom line
Scorecard at a glanceFully disclosed formula with clinical evidence supporting its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
This product combines several traditional cleansing herbs but carries real interaction risks, especially if you take blood thinners, diabetes medications, diuretics, heart drugs, or seizure medications. The evidence that these ingredients work is mixed or absent for most uses.
Talk with your pharmacist before starting, especially if you're on any prescription medications or planning pregnancy or breastfeeding.

Super Cleans-R by Purium Drug Interactions
HelloPharmacist Interaction Report
Super Cleans-R by Purium contains seven active ingredients, several of which interact with medications.
The most serious concern is wormwood, which moderately interacts with anticonvulsants—the herb's thujone content can interfere with seizure control, so anyone on these drugs should avoid this product.
Read the full breakdown — every affected drug type, severity by severity
Multiple ingredients raise moderate-severity concerns with blood thinners and antiplatelet drugs. Marshmallow flower may have antiplatelet effects, Indian gooseberry (amalaki) reduces platelet aggregation and increases bleeding time, and clove oil has antiplatelet activity.
If you take aspirin, clopidogrel (Plavix), or other anticoagulants, this product could compound bleeding risk. Cascara sagrada and senna both interact with warfarin by potentially increasing its effects through diarrhea.
Clove's main constituent, eugenol, moderately inhibits several liver enzymes (CYP2D6, CYP1A2, CYP3A4, and CYP2C9), which could raise levels of many prescription drugs metabolized by these pathways—though this effect has not been confirmed in humans. Indian gooseberry and clove both moderately interact with diabetes medications and may increase low blood sugar (hypoglycemia) risk.
Cascara sagrada and senna also moderately interact with diuretics and corticosteroids due to potassium loss, and both interact with digoxin (a heart drug) through the same mechanism.
Altogether, these interactions span 2,125 individual medications. We could not check three ingredients—black walnut, wormwood, and senna do not have complete interaction data on file.

Marshmallow
Lithium
Theoretically, due to potential diuretic effects, marshmallow might reduce excretion and increase levels of lithium.
Marshmallow is thought to have diuretic properties. To avoid lithium toxicity, the dose of lithium might need to be decreased when used with marshmallow.
Anticoagulant/Antiplatelet Drugs
Theoretically, marshmallow flower might have antiplatelet effects.
Animal research suggests that marshmallow flower extract has antiplatelet effects. However, the root and leaf of marshmallow, not the flower, are the plant parts most commonly found in dietary supplements. Theoretically, use of marshmallow flower with anticoagulant/antiplatelet drugs can have additive effects, and might increase the risk for bleeding in some patients.
Oral Drugs
Theoretically, mucilage in marshmallow might impair absorption of oral drugs.
Marshmallow contains mucilage which can affect oral drug absorption. To avoid changes in absorption, take marshmallow 30-60 minutes after oral medications.
Clove
Antidiabetes Drugs
Theoretically, concomitant use of clove extracts with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical and laboratory research suggest that polyphenol extracts from clove flower buds might lower blood glucose levels. Dosing adjustments for insulin or oral hypoglycemic agents may be necessary when taken with clove. Monitor blood glucose levels closely.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, concomitant use of clove may increase levels of drugs metabolized by CYP1A2.
In vitro research shows that eugenol, the principal constituent of clove, can inhibit CYP1A2 in a dose-dependent manner,. This effect has not been reported in humans.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, concomitant use of clove may increase levels of drugs metabolized by CYP2C9.
In vitro research shows that eugenol, the principal constituent of clove, inhibits CYP2C9 in a dose-dependent manner. This effect has not been reported in humans.
Cytochrome P450 2D6 (Cyp2D6) Substrates
Theoretically, concomitant use of clove may increase levels of drugs metabolized by CYP2D6.
In vitro research shows that eugenol, the principal constituent of clove, can inhibit CYP2D6 in a dose-dependent manner. This effect has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, concomitant use of clove may increase levels of drugs metabolized by CYP3A4.
In vitro research shows that eugenol, the principal constituent of clove, can inhibit CYP3A4 in a dose-dependent manner. This effect has not been reported in humans.
Anticoagulant/Antiplatelet Drugs
Theoretically, clove oil may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Laboratory research suggests that eugenol, a constituent of clove, has antiplatelet activity. This interaction has not been reported in humans.
Ibuprofen (Advil, Others)
Theoretically, topical application of clove oil with ibuprofen might increase the absorption and side effects of topical ibuprofen.
Laboratory research shows that topical application of clove oil increases the absorption of topical ibuprofen. This interaction has not been reported in humans.
Cascara Sagrada
Corticosteroids
Theoretically, cascara sagrada might increase the risk of hypokalemia when taken with corticosteroids.
Cascara sagrada has stimulant laxative effects, and long-term use has been associated with hypokalemia.
Digoxin (Lanoxin)
Theoretically, cascara sagrada might cause hypokalemia, potentially increasing the risk of digoxin toxicity.
Cascara sagrada has stimulant laxative effects, and long-term use has been associated with hypokalemia.
Diuretic Drugs
Theoretically, cascara sagrada might increase the risk of hypokalemia when taken with diuretic drugs.
Cascara sagrada has stimulant laxative effects, and long-term use has been associated with hypokalemia.
Stimulant Laxatives
Theoretically, cascara sagrada might have additive adverse effects when taken with stimulant laxatives.
Cascara sagrada has stimulant laxative effects and might compound fluid and electrolyte losses when taken with stimulant laxatives.
Warfarin (Coumadin)
Theoretically, cascara sagrada might increase the risk of bleeding when taken with warfarin.
Cascara sagrada has stimulant laxative effects. In some people, cascara sagrada can cause diarrhea. Diarrhea can increase the effects of warfarin, increase international normalized ratio (INR), and increase the risk of bleeding.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.
Amalaki
Anticoagulant/Antiplatelet Drugs
Theoretically, Indian gooseberry may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs; however, research is conflicting.
Clinical research shows that taking Indian gooseberry 500 mg as a single dose or twice daily for 10 days reduces platelet aggregation by about 24% to 36%, increases bleeding time by about 3.8-5.9 seconds, and increases clotting time by about 9.8-12.7 seconds when compared to baseline. However, taking Indian gooseberry 500 mg along with clopidogrel 75 mg or ecosprin 75 mg, as a single dose or for 10 days, does not significantly reduce platelet aggregation or increase bleeding time or clotting time when compared with clopidogrel 75 mg or ecosprin 75 mg alone. Until more is known, use caution when taking Indian gooseberry in combination with anticoagulant/antiplatelet drugs.
Antidiabetes Drugs
Taking Indian gooseberry with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical research shows that taking Indian gooseberry fruit or fruit extract alone or in conjunction with antidiabetes medications can lower blood glucose levels. Dose adjustments to diabetes medications might be necessary.
Aspirin
Theoretically, Indian gooseberry may increase the risk of bleeding if used with aspirin; however, research is conflicting.
However, taking a single dose of Indian gooseberry 500 mg along with ecosprin 75 mg, or taking a combination of Indian gooseberry 500 mg twice daily plus ecosprin 75 mg once daily for 10 days, does not significantly reduce platelet aggregation or increase bleeding time or clotting time when compared with ecosprin 75 mg alone.
Clopidogrel (Plavix)
Theoretically, Indian gooseberry may increase the risk of bleeding if used with clopidogrel; however, research is conflicting.
Senna
Digoxin (Lanoxin)
Theoretically, senna might increase the risk of adverse effects when taken with digoxin.
Overuse/abuse of senna increases the risk of adverse effects from cardiac glycosides, such as digoxin, due to potassium depletion.
Diuretic Drugs
Theoretically, senna might increase the risk of hypokalemia when taken with diuretic drugs.
Overuse of senna might compound diuretic-induced potassium loss and increase the risk for hypokalemia.
Estrogens
Theoretically, taking senna may interfere with the absorption of exogenous estrogens.
Some preliminary clinical evidence suggests that senna reduces the absorption of estradiol and decreases serum concentrations of estrone and estrone sulfate by decreasing intestinal transit time.
Stimulant Laxatives
Theoretically, senna might increase the risk for fluid and electrolyte loss when taken with other stimulant laxatives.
Senna is a stimulant laxative; concomitant use with other stimulant laxatives might compound fluid and electrolyte loss.
Warfarin (Coumadin)
Theoretically, excessive use of senna might increase the effects of warfarin.
Senna has stimulant laxative effects and can cause diarrhea. Diarrhea can increase the effects of warfarin, increase international normalized ratio (INR), and increase the risk of bleeding. In one case report, excessive use of senna for 3 weeks resulted in diarrhea, bloody stools, and an elevated INR of 11.9.
Wormwood
Anticonvulsants
Theoretically, taking wormwood might interfere with the effects of anticonvulsant drugs.
Thujone, a constituent of wormwood, has convulsant effects.
