COENZYME Q10

ANTIDIABETES DRUGS

Metformin and sulfonylureas can modestly decrease endogenous levels of coenzyme Q10, but coenzyme Q10 supplements are not likely to be needed for patients taking these drugs.

Some antidiabetes drugs, including metformin and sulfonylureas, appear to inhibit coenzyme Q10 enzyme activity and decrease endogenous levels of coenzyme Q10. However, there is no strong evidence that coenzyme Q10 depletion is the cause of antidiabetes drug-related side effects. Furthermore, taking coenzyme Q10 does not seem to improve fasting glucose levels or glycated hemoglobin in patients with diabetes.

There is no routine blood test for CoQ10 in everyday practice. If you have new, unexplained muscle symptoms on a medication linked to CoQ10 (statins are the best-known example), talk to your prescriber before changing anything — symptoms have many possible causes and your prescriber has several options.

CoQ10 is generally well tolerated, but evidence for supplementing (for example, for statin muscle symptoms) is mixed. It may also modestly affect warfarin — check with your pharmacist before combining.

METFORMIN (Glucophage)

Metformin might reduce serum folate levels and increase the risk of folate deficiency.

Reduced levels of vitamin B12 and, to a lesser extent, folic acid occur in some people with diabetes and can contribute to hyperhomocysteinemia, adding to their already increased risk of cardiovascular disease. The reduced folate levels seen in people with diabetes have been linked to metformin use in some cases, possibly as a result of reduced folic acid absorption. Symptomatic folate deficiency is unlikely to occur with metformin, but people with diabetes may need folic acid supplements to reduce hyperhomocysteinemia. VITAMIN B12

METFORMIN (Glucophage)

Metformin reduces vitamin B12 levels in a dose and duration-dependent manner, which might result in deficiency.

Metformin might interfere with intrinsic factor secretion or uptake, affect bowel motility, or cause bacterial overgrowth, resulting in vitamin B12 malabsorption and hyperhomocysteinemia. There are also rare reports of megaloblastic anemia in people who have taken metformin for 5 years or more. Reduced absorption occurs in up to 30% of people taking metformin chronically and appears to be a dose-dependent effect. However, clinically significant deficiency isn't likely to develop if dietary intake of vitamin B12 is adequate, including the intake of a multivitamin or vitamin B12 supplement. The risk of deficiency is higher in those taking a higher metformin dose for 3 years or more and also in older people and vegetarians. Deficiency can be corrected with vitamin B12 supplements even if metformin is continued. Monitor for signs and symptoms of vitamin B12 and folic acid deficiency. Advise people taking metformin chronically to include adequate amounts of vitamin B12 in the diet and to have serum vitamin B12 and homocysteine levels checked annually. What Vitamin B12 does & food sources

Vitamin B12 supports red blood cell formation, nerve function, and DNA synthesis.

THIAMINE

Insufficient Evidence

METFORMIN (Glucophage)

Theoretically, metformin might reduce thiamine activity.
Thiamine is involved in the entrance of glycolytic pyruvate into the mitochondrial Kreb's cycle. Reducing thiamine activity might lead to increased amounts of pyruvate being converted to lactic acid. This effect might be responsible for some of the cases of lactic acidosis associated with metformin. This theoretical effect is based on preclinical animal research with phenformin, a similar drug that was produced prior to metformin. This has not yet been substantiated in humans or with the use of metformin.

Can medications really deplete nutrients?
Yes — some medications can lower levels of certain vitamins, minerals, or other nutrients over time, by reducing absorption, increasing loss through the kidneys or gut, or changing how the body uses them. Well-known examples include metformin (vitamin B12) and proton pump inhibitors like omeprazole (magnesium and B12). How much it matters varies a lot between medications and between people.
