Increased dofetilide exposure and an increased risk of QT interval prolongation

Interaction Deep Dive

Avoid coadministration of dofetilide and nilotinib, both drugs that prolong the QT interval, due to the potential for additive effects on the QT interval and increased risk of torsade de pointes1Concomitant use of dofetilide and nilotinib (CYP3A4 inhibitor) may also increase dofetilide exposure 2. However, if coadministration is required, closely monitor the patient for prolongation of the QT interval 3. Obtain ECGs periodically to monitor the QTc at baseline, seven days after initiation, and periodically thereafter, and adjust the dose accordingly 4.
