Olaparib is a CYP3A4 substrate and in a pharmacokinetic simulation, coadministration with fluconazole (a moderate CYP3A4 inhibitor) suggested an important increase in olaparib exposure will occur. Avoid concomitant use of olaparib and a moderate CYP3A4 inhibitor, and consider an alternate agent with less CYP3A inhibition. If coadministration is necessary, reduce olaparib tablet dosage to 150 mg twice daily1.
