Macitentan, Tadalafil falls under ORAL DRUGS — the group Agar interacts with.

Agar forms a thick gel, which may disrupt the absorption of nutrients and medications. Animal research suggests that consuming a diet consisting of 5% to 10% agar alters the absorption of nutrients from the diet. In one study, consuming a diet composed of 10% agar reduced the absorption of all evaluated minerals. Theoretically, agar may also impair the absorption of oral drugs.

Here's the simple version: Opsynvi combines two blood pressure medicines (macitentan and tadalafil) that treat pulmonary arterial hypertension. Agar is a seaweed fiber that swells into a gel in your gut. In theory, that gel could grab onto pills you swallow around the same time and slow or reduce how much drug your body absorbs.

If less of your Opsynvi gets absorbed, it may not work as well at keeping your lung pressures down. I'll be honest with you though, this concern is theoretical and comes mostly from animal studies using very high-agar diets. Occasional food-grade agar is unlikely to matter. If you're using agar as a fiber supplement, a good habit is to space it a couple of hours apart from your Opsynvi and to check with your pharmacist first.
