Isosorbide Mononitrate falls under ORAL DRUGS — the group Agar interacts with.

Agar forms a thick gel, which may disrupt the absorption of nutrients and medications. Animal research suggests that consuming a diet consisting of 5% to 10% agar alters the absorption of nutrients from the diet. In one study, consuming a diet composed of 10% agar reduced the absorption of all evaluated minerals. Theoretically, agar may also impair the absorption of oral drugs.

Here's the gist with isosorbide mononitrate: because agar swells into a thick gel in your stomach and intestines, the worry is that it could physically trap a little of your tablet and slow or reduce how much gets absorbed. With a nitrate, less absorption could mean it doesn't prevent your angina (chest pain) as well as it should. This is a theory based on how agar behaves and on animal feeding studies, not proven in people taking this medicine, so the real-world risk is likely small.

To play it safe, don't take your isosorbide at the exact same time as a big agar dose; separating them by a couple of hours helps. If your chest pain creeps back or your dosing changes, check with your pharmacist or doctor before adjusting anything.

Isosorbide is used to prevent angina pectoris — chest pain or tightness caused by coronary artery disease. Both isosorbide dinitrate tablets and isosorbide mononitrate tablets (including Monoket and Isordil) are used for this purpose.

Important: none of these oral formulations work fast enough to stop a chest pain episode that has already started. They are taken on a schedule to prevent angina, not to treat it in the moment.
