Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here's the gist for these two: the concern on file is really about the thyroid extract being absorbed properly, not about your isosorbide changing. When you swallow something alongside thyroid hormone, certain pills and foods can grab onto the hormone in your gut and keep your body from taking it in well. So the practical worry is your thyroid supplement working less reliably if you take everything at the same moment.

Isosorbide mononitrate itself is a vasodilator and isn't a prodrug here, so I wouldn't expect it to make your nitrate work harder or weaker in a dangerous way. To be safe, simply separate them by a few hours. Since over-the-counter thyroid products are unpredictable anyway, please loop in your doctor or pharmacist about the best timing.

What Is Isosorbide Used For?

Isosorbide is used to prevent angina pectoris — chest pain or tightness caused by coronary artery disease. Both isosorbide dinitrate tablets and isosorbide mononitrate tablets (including Monoket and Isordil) are used for this purpose.

Important: none of these oral formulations work fast enough to stop a chest pain episode that has already started. They are taken on a schedule to prevent angina, not to treat it in the moment.
