Thyroid Extract and Fenofibrate

Here is the interaction we found between these two, with the evidence behind it.

Minor

Oral Drugs

Why this applies to Fenofibrate

Fenofibrate falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.
Severity Minor Likelihood Likely Evidence A

Well-controlled human studies have demonstrated the likely existence of this interaction.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here the concern runs the other direction from what you might expect. It's not that fenofibrate (Tricor) changes your thyroid extract. Instead, taking other things alongside thyroid hormone can lower how much of that hormone your gut actually absorbs. Fenofibrate is a cholesterol and triglyceride medicine taken with food, and having it in your stomach at the same time as your thyroid product may blunt the thyroid absorption a bit.

The good news is this is rated minor and is easy to manage with timing. Thyroid hormone works best on an empty stomach, so separating it from fenofibrate and meals usually solves it. Since over-the-counter thyroid extract is unpredictable, please loop in your doctor or pharmacist about the best schedule for you.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

From our pharmacist’s overview on the Fenofibrate page

“Fenofibrate (Tricor, Antara) is a cholesterol and triglyceride medicine that can strain the liver on its own, so this list of 59 supplements deserves genuine care rather than a shrug. Most of it, 38 ingredients, involves herbs that also stress the liver, ashwagandha among them, and stacking those effects is the risk that matters most h…”

About Thyroid Extract

What is Thyroid Extract?

Thyroid Extract
  • Hypothyroidism (underactive thyroid)
  • Fatigue and low energy
  • Weight management
  • Sluggish metabolism
  • General thyroid support

Thyroid extract supplements are made from dried animal thyroid gland and may contain real thyroid hormones (T3 and T4), but the amounts in over-the-counter products are not standardized and can be unpredictable. Self-treating a thyroid problem with these supplements can be risky, so always work with a doctor or pharmacist and use prescription thyroid medicine when treatment is needed.

About Fenofibrate

What Is Fenofibrate Used For?

Brand names: Tricor, Fenofibrate, Lipidil Micro, Lipidil Supra, Antara, Lofibra, Triglide, Lipidil EZ

Fenofibrate is used along with diet to improve blood fats in adults. Tricor and the fenofibrate tablets are used to lower LDL (“bad”) cholesterol, total cholesterol, triglycerides and apolipoprotein B, and to raise HDL (“good”) cholesterol in primary hypercholesterolemia (high cholesterol not caused by another condition) or mixed dyslipidemia. They are also used for severe hypertriglyceridemia (very high triglycerides).

Antara and Lipofen capsules are used to lower triglycerides in adults with severe hypertriglyceridemia (500 mg/dL or higher). They are also used to lower LDL cholesterol in primary hyperlipidemia (high blood fats not caused by another condition) when the recommended LDL-lowering therapy is not possible. Fenofibrate did not reduce heart disease complications or death in people with type 2 diabetes in large trials.
