Thyroid Extract and Nitroglycerin

Here is the interaction we found between these two, with the evidence behind it.

Minor

Oral Drugs

Why this applies to Nitroglycerin

Nitroglycerin falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.
Severity Minor Likelihood Likely Evidence A

Well-controlled human studies have demonstrated the likely existence of this interaction.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: this interaction is really about how well your body absorbs the thyroid hormone, not about the nitroglycerin itself. When you take an oral pill close to your thyroid extract, the two can mix in your stomach and the thyroid hormone may not get absorbed as well. That can leave your thyroid treatment working less than it should over time.

The good news is that nitroglycerin you take for chest pain is usually dosed as a tablet under the tongue, a spray, an ointment, or a patch, so it often doesn't share stomach space with a swallowed thyroid product. If you do take any oral form, simple spacing usually fixes it. A common trick is taking thyroid on an empty stomach and other pills later. Run the timing past your pharmacist to be safe.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Nitroglycerin page

“Nitroglycerin (Nitrostat, Nitro-Bid) exists to open blood vessels quickly during angina, so a supplement that also drops blood pressure, or one that fights the drug's effect, is not a small matter, and this list warrants real care before you add to it. The most serious entries pull in two directions.”

About Thyroid Extract

What is Thyroid Extract?

Thyroid Extract
  • Hypothyroidism (underactive thyroid)
  • Fatigue and low energy
  • Weight management
  • Sluggish metabolism
  • General thyroid support

Thyroid extract supplements are made from dried animal thyroid gland and may contain real thyroid hormones (T3 and T4), but the amounts in over-the-counter products are not standardized and can be unpredictable. Self-treating a thyroid problem with these supplements can be risky, so always work with a doctor or pharmacist and use prescription thyroid medicine when treatment is needed.
