Here is the interaction we found between these two, with the evidence behind it.
Oral Drugs
Why this applies to Pancrelipase (prescription drug)
Pancrelipase (prescription drug) falls under Oral Drugs — the group Thyroid Extract interacts with.
Well-controlled human studies have demonstrated the likely existence of this interaction.
Here's the simple version: your thyroid extract works best when it has your stomach to itself. Pancrelipase is a digestive enzyme product you take with meals, so taking the two at the same time can get in the way of how well the thyroid hormone is absorbed. When absorption drops, you may not get the full thyroid effect, which over time can mean lingering symptoms like fatigue, feeling cold, or sluggishness.
The good news is this is a minor, timing-related issue, and spacing them apart usually solves it. A common approach is to take the thyroid product on an empty stomach, at least 30 to 60 minutes before eating and before your pancrelipase dose. Please confirm the right timing with your pharmacist or doctor before changing anything.
What These Ratings Mean
Every interaction above is graded three ways — here’s the key to each label.
Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.

From our pharmacist’s overview on the Pancrelipase (prescription drug) page
“Pancrelipase (Viokace, Pancrease, Cotazym) replaces digestive enzymes a failing pancreas cannot supply, and since it does all of its work inside the gut, the 19 supplement interactions on file deserve real attention rather than a shrug. Most involve fibers, gums, and mucilage-rich herbs that can slow or reduce absorption of oral medici…”

About Thyroid Extract
What is Thyroid Extract?
- Hypothyroidism (underactive thyroid)
- Fatigue and low energy
- Weight management
- Sluggish metabolism
- General thyroid support
Thyroid extract supplements are made from dried animal thyroid gland and may contain real thyroid hormones (T3 and T4), but the amounts in over-the-counter products are not standardized and can be unpredictable. Self-treating a thyroid problem with these supplements can be risky, so always work with a doctor or pharmacist and use prescription thyroid medicine when treatment is needed.
About Pancrelipase
What Is Pancrelipase Used For?
Brand names: Viokace, Cotazym ECS 4, Pancrease, Cotazym ECS 65B, Cotazym ECS 8, Cotazym, Cotazym ECS 20, CREON, Ultrase MT, Ultrase, Pertzye, Pancreaze, Zenpep
Pancrelipase is used to treat exocrine pancreatic insufficiency (EPI) — a condition where the pancreas doesn't produce enough digestive enzymes. Without these enzymes, the body struggles to break down and absorb nutrients from food.
It is indicated for adults with EPI caused by chronic pancreatitis (long-term pancreas inflammation) or pancreatectomy (surgical removal of the pancreas, in whole or in part). It is also used in people with cystic fibrosis who develop EPI.
