Here's the gist with tragacanth and your isosorbide mononitrate (Imdur and similar brands). Tragacanth is basically a gel-forming fiber. When it swells up in your stomach and gut, it can grab onto pills you swallow around the same time and slightly change how much medicine your body soaks up. For a heart pill like this one, that's worth being mindful of, because steady absorption is what keeps your angina (chest pain) under control.

This is a theoretical concern based on how the fiber behaves, not something proven in real patients, so I wouldn't lose sleep over it. The simple fix: take your isosorbide first, then wait 30 to 60 minutes before taking tragacanth. If you notice more chest pain, or extra dizziness or headache, check in with your pharmacist or doctor before changing anything.

There is very little solid human research showing it treats any specific condition. It is generally considered safe in food amounts, but anyone with allergies or who is pregnant or breastfeeding should check with a pharmacist or doctor first.

Isosorbide is used to prevent angina pectoris — chest pain or tightness caused by coronary artery disease. Both isosorbide dinitrate tablets and isosorbide mononitrate tablets (including Monoket and Isordil) are used for this purpose.

Important: none of these oral formulations work fast enough to stop a chest pain episode that has already started. They are taken on a schedule to prevent angina, not to treat it in the moment.
