Here's the simple version: tragacanth is a plant gum that swells into a thick, slippery gel when it hits fluid. When you swallow it alongside Pepto-Bismol or another bismuth subsalicylate product, that gel could trap some of the medicine and change how much your gut soaks up. It might slow it down, lessen it, or have no real effect. This is a theoretical concern based on how the fiber behaves, not something proven in studies, so I wouldn't lose sleep over it.

The practical worry would be slightly weaker relief from your diarrhea, heartburn, or upset stomach. An easy fix is spacing them out: take your bismuth first, then wait about 30 to 60 minutes before the tragacanth. If you have questions, your pharmacist is happy to help.

What Is Bismuth Subsalicylate Used For?

Brand names: Pepto-Bismol, Kaopectate Anti-Diarrheal Caplets (U.S.), Maalox Total Stomach Relief, Maalox Multi Action, Kaopectate Liquid (U.S.), Kaopectate Extra Strength Liquid (U.S.)

Bismuth subsalicylate is used for diarrhea and traveler's diarrhea. It also relieves upset stomach from overindulgence in food and drink.

That upset stomach can show up as heartburn, indigestion, nausea, gas, belching or a feeling of fullness. A few products (such as some Rite Aid, WinCo, DRx Choice regular strength suspensions and Stomach Relief Maximum Strength liquid) list the symptoms without “fullness.”
