Diltiazem falls under ORAL DRUGS — the group Tragacanth interacts with.
Tragacanth contains hydrocolloidal fiber which can decrease, increase, or not affect oral drug absorption. To avoid changes in absorption, take tragacanth 30-60 minutes after oral medications.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with tragacanth and your diltiazem. Tragacanth is mostly a thick, gel-forming fiber. When you swallow it, that slippery gel can sit in your gut alongside your pill and potentially get in the way of how much diltiazem your body soaks up. It could nudge absorption down, up, or not at all, so the direction isn't certain. If less gets absorbed, your blood pressure or chest pain might be less controlled. If more sneaks in, you could feel more lightheaded, flushed, or notice a slower heartbeat or swollen ankles.
To be safe, simply space them out: take your diltiazem first, then wait about 30 to 60 minutes before any tragacanth. This is theoretical, based on how the fiber behaves, not on strong studies, so don't panic. Just run it by your pharmacist to be sure.

Tragacanth is a plant gum used mainly as a thickener and emulsifier in foods, cosmetics, and medicines, and traditionally to soothe the throat and gut. There is very little solid human research showing it treats any specific condition. It is generally considered safe in food amounts, but anyone with allergies or who is pregnant or breastfeeding should check with a pharmacist or doctor first.

Oral extended-release diltiazem (capsules and tablets) treats high blood pressure (hypertension). It is also used for chronic stable angina, which is chest pain from the heart working harder than its blood supply allows. It can be used alone or with other blood pressure medicines.
Some extended-release capsule products, such as Cartia, Cartia Xt and certain generic diltiazem capsules, are also approved for angina due to coronary artery spasm. The intravenous (IV) injection is a different use: it gives temporary control of a rapid heart rate in atrial fibrillation or atrial flutter, and rapidly converts paroxysmal supraventricular tachycardia (PSVT) to normal rhythm.
