Why this applies to Isosorbide Mononitrate

Isosorbide Mononitrate falls under ORAL DRUGS — the group Black Psyllium interacts with.

Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin. Although many of these studies evaluated blond psyllium, the fiber content in black psyllium may have similar effects.

Here's the simple version: black psyllium is a soluble fiber that turns into a thick gel in your gut. That gel can sometimes grab onto medications you swallow and change how much your body absorbs. With isosorbide mononitrate, the worry is mostly theoretical, and the effect could go either way (a little more, a little less, or no change at all). Isosorbide is not a prodrug, so we're only talking about absorption, not activation.

If absorption dipped, your angina protection might not work as well; if it rose, you could notice more headache, dizziness, or lightheadedness. An easy fix is timing: take your isosorbide first, then wait about 30 to 60 minutes before your psyllium. Always drink plenty of water with fiber, and check with your pharmacist if you have questions.

Black psyllium is a soluble-fiber supplement made from the seeds of a Plantago plant, used mostly to ease constipation and support digestive health. It is best known and most studied for relieving constipation and may modestly help lower cholesterol, but it must always be taken with plenty of water. Talk with your pharmacist or doctor before starting, especially if you have swallowing problems or bowel disorders.

What Is Isosorbide Used For?

Brand names: Imdur, Angeze, Dynamin, Elantan, Ismo, Chemydur 60XL, Elantan LA, Isib 60XL, Ismo Retard, Isodur, Isotard, Modisal LA, Modisal XL, Monomax SR, Monomax XL, Angeze SR, Monomil XL, Monosorb XL, Zemon

Isosorbide is used to prevent angina pectoris — chest pain or tightness caused by coronary artery disease. Both isosorbide dinitrate tablets and isosorbide mononitrate tablets (including Monoket and Isordil) are used for this purpose.

Important: none of these oral formulations work fast enough to stop a chest pain episode that has already started. They are taken on a schedule to prevent angina, not to treat it in the moment.
