Black Psyllium interacts with whole categories of medication. Metformin Hydrochloride falls into 2 of those categories, so it can interact with Black Psyllium in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
METFORMIN (Glucophage)
1 of 2Animal research shows that concurrent consumption of blond psyllium with metformin slows and increases the absorption of metformin. This interaction may also occur with black psyllium. To avoid changes in absorption, take psyllium 30-60 minutes after metformin.

Here's the gist: when you take black psyllium and your metformin (Riomet ER) at the same time, that thick fiber gel may slow down and slightly change how much metformin your body absorbs. The thinking is this could nudge metformin's effects up a bit, both the blood-sugar lowering and the typical side effects like nausea, stomach upset, diarrhea, or a higher chance of low blood sugar if you're also on other diabetes meds.
I'd keep this in perspective though. This is based only on animal research, so it's more of a theoretical concern than something proven in people. A simple fix is to space them out: take your metformin first, then your psyllium about 30 to 60 minutes later. Check with your pharmacist or doctor before starting psyllium.

Metformin Hydrochloride falls under ORAL DRUGS — the group Black Psyllium interacts with.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin. Although many of these studies evaluated blond psyllium, the fiber content in black psyllium may have similar effects.

Here's the simple version: that thick gel black psyllium forms in your gut can change how your body soaks up your metformin. The research here is limited and mostly from animal studies, so this isn't a big alarm. In those studies, psyllium actually slowed down but slightly increased the overall amount of metformin absorbed. In real life this usually doesn't cause problems, but it could nudge your blood sugar around or add to typical metformin tummy upset like nausea, gas, or loose stools.
The easy fix is timing. Take your metformin first, then wait about 30 to 60 minutes before your psyllium, and drink plenty of water. If you notice low blood sugar symptoms or worse stomach issues, check in with your pharmacist or doctor before changing anything.
