Why this applies to Metformin Hydrochloride

Theoretically, taking acai with antidiabetes drugs might interfere with glycemic control.
Preliminary clinical research in healthy adults has shown that taking acai may increase or decrease levels of fasting blood glucose.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: metformin works to keep your blood sugar in a healthy range, and acai supplements might nudge that blood sugar in either direction. Some early research in healthy people showed acai could raise or lower fasting blood sugar, so it's hard to predict which way it would go for you. This isn't about acai changing how your body breaks down metformin; it's more that the berry may have its own small effect on glucose.

The good news is this concern is mostly theoretical and based on how acai behaves, not on strong proof of harm. Enjoying acai as a food in a smoothie bowl is very unlikely to be an issue. With concentrated acai supplements, it's worth keeping an eye on your blood sugar readings and mentioning the supplement to your pharmacist or doctor before adding it.

From our pharmacist’s overview on the Metformin Hydrochloride page

“Anyone taking metformin hydrochloride (Riomet ER) should treat this list with genuine care, because two of its 257 entries are rated major. Siberian cocklebur has caused severe hypoglycemia in people and should not be combined with metformin at all, and activated charcoal can adsorb the drug in the gut so less reaches your blood.”
