Here's the honest picture with acai and your Metformin XR: this is a theoretical concern, not something proven to cause problems. Metformin works to lower your blood sugar, and it isn't a prodrug, so we aren't worried about acai changing how your body activates it. The real question is whether acai itself nudges your blood sugar a little.

Some early research in healthy people found acai could raise or lower fasting blood sugar, but the direction wasn't consistent. Eating acai as a food (a smoothie or a bowl) is very unlikely to matter. Concentrated acai supplements are the bigger unknown. If you decide to use them regularly, just keep an eye on your home glucose readings and mention it to your pharmacist or doctor so they can help you watch for any unexpected highs or lows.
