Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing with acai and repaglinide (Prandin): repaglinide already works to lower your blood sugar by nudging your pancreas to release insulin. The concern on file is that acai supplements might shift your fasting blood sugar in either direction, so layering them together could make your numbers less predictable. If acai pushes things a little lower, you might be at higher risk for a low blood sugar episode, meaning shakiness, sweating, dizziness, or feeling foggy.

I want to be honest, though: this is theoretical, based mostly on how things work in the lab and a little early human research, not strong proof. Enjoying acai as a food is very unlikely to cause trouble. If you're taking concentrated acai supplements, just give your pharmacist or doctor a heads-up and keep an eye on your glucose readings.

Nearly everything on it also acts on blood sugar, usually pushing it lower still and raising the odds of a hypoglycemic crash, though a handful, niacin among them, pull the other way a…”
