Theoretically, black psyllium might increase the therapeutic and adverse effects of metformin.
Animal research shows that concurrent consumption of blond psyllium with metformin slows and increases the absorption of metformin. This interaction may also occur with black psyllium. To avoid changes in absorption, take psyllium 30-60 minutes after metformin.

Your ActoPlus Met combines two diabetes medicines: pioglitazone and metformin. The interaction we have on file involves the metformin part. Black psyllium forms a thick gel when it hits water, and that gel can slow how metformin gets absorbed and may pull a bit more of it into your system over time. In theory, that could nudge your blood sugar lower than expected and bump up metformin's usual side effects, like nausea, an upset stomach, or loose stools.

I want to be honest though: this is based only on animal research, so it is more of a caution than a proven problem. A simple fix is to space them out, taking your psyllium about 30 to 60 minutes after your medicine. Check with your pharmacist if you watch your sugars closely.

pioglitazone, metformin falls under ORAL DRUGS — the group Black Psyllium interacts with.

Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin. Although many of these studies evaluated blond psyllium, the fiber content in black psyllium may have similar effects.

Here's the simple version: when you take black psyllium, that soluble fiber forms a thick gel in your gut. That gel can grab onto medications and change how much your body soaks up, and sometimes how quickly. For your ActoPlus Met (pioglitazone plus metformin), the research is pretty mild. In fact, some animal studies suggest psyllium may actually slow down but slightly increase the amount of metformin absorbed. Neither of these drugs is a prodrug, so the worry is really just about timing throwing off your blood sugar control.

The practical fix is easy. Take your ActoPlus Met first, then wait about 30 to 60 minutes before your psyllium. The evidence here is fairly weak and the interaction is minor, so don't panic, but do run it by your pharmacist or doctor so your blood sugar stays steady.
