Here's the practical picture with Synalgos DC (aspirin, caffeine, and dihydrocodeine). The piece of this combo that runs through the CYP3A4 pathway is mainly the dihydrocodeine, which is the opioid pain reliever. Some studies hint that honey may slightly speed up (induce) CYP3A4, which could in theory clear that opioid a little faster and trim how much pain relief you feel. But honestly, the research here is mixed and weak, and different honeys behave differently.

This is rated a minor, possible interaction, so the normal amount of honey you'd put in tea or on toast is very unlikely to throw off your medicine. I wouldn't lose sleep over it. If you're using large, concentrated honey supplements daily, just mention it to your pharmacist or doctor so they can keep an eye on your pain control.

Theoretically, honey may increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
In vitro, honey inhibits platelet aggregation and increases the time to clotting. Furthermore, animal research suggests that feeding mice large doses of honey for 12 days increases bleeding time when compared with no intervention. However, these effects have not been reported in humans.

Here's the honest picture with Synalgos-DC (aspirin, caffeine, and dihydrocodeine) and honey. The aspirin part already thins your blood a bit by keeping platelets from sticking together. In lab dishes and in mice fed large amounts, honey seemed to do a little of the same thing, slightly slowing clotting. So in theory, the two together could nudge your bleeding risk up, meaning things like easier bruising, nosebleeds, or a cut that takes longer to stop.

That said, this has never actually been seen in people, and the evidence is very weak. Normal amounts of honey on toast or in tea are very unlikely to matter. If you take honey in large, concentrated supplement doses or notice unusual bruising or bleeding, check in with your pharmacist or doctor.
