Iloprost falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Honey interacts with.

Theoretically, honey may increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
In vitro, honey inhibits platelet aggregation and increases the time to clotting. Furthermore, animal research suggests that feeding mice large doses of honey for 12 days increases bleeding time when compared with no intervention. However, these effects have not been reported in humans.

Here's the gist for folks using iloprost (Ventavis). Besides relaxing blood vessels in your lungs, iloprost also gently discourages platelets (your blood's clotting cells) from sticking together. Honey, in lab and animal studies, seems to do a little of the same thing. So in theory, pairing them could nudge your bleeding risk up slightly, showing up as things like easier bruising, nosebleeds, or bleeding gums.

That said, I don't want to overstate this. The evidence is only from test tubes and mice fed large amounts of honey, and nothing like this has actually been seen in people. Normal food amounts of honey are very unlikely to matter. Still, if you bruise or bleed more than usual, mention it to your pharmacist or doctor.

Specifically, it is used to reduce the risk of losing fingers or toes (digit amputation) after serious frostbite injury. The clinical evidence supporting this use comes from young, healthy adults who suffered frostbite at high altitudes.
