Why this applies to Iloprost

Iloprost falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Danshen interacts with.

Theoretically, danshen may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Danshen has been reported to have antithrombotic effects. Animal research also suggests that taking a danshen combination formula with clopidogrel exhibits a synergistic increase in antiplatelet aggregation and prolongation of coagulation time when compared with either taken alone.

Here's something worth knowing if you inhale iloprost (Ventavis) for your lung pressure. Besides relaxing blood vessels, iloprost also makes your platelets less sticky, so it has a mild blood-thinning side to it. Danshen seems to do something similar, as it may slow down clotting on its own. Put the two together and those effects can stack up, which could raise your chance of bruising easily, nosebleeds, bleeding gums, or bleeding that's slower to stop.

To be fair, this is based mostly on animal studies and scattered reports, not strong human research, so it's a theoretical concern rather than a proven one. Still, bleeding is serious enough that I'd loop in your pharmacist or doctor before adding Danshen.

Theoretically, taking danshen with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.

Here's the gist for Ventavis (iloprost): it's a vasodilator, meaning it works by relaxing and widening your blood vessels. Danshen seems to do something similar. In animal studies, it lowered blood pressure on its own, and it made another blood pressure drug work even harder. So the worry is simple addition. Stacking Danshen on top of iloprost could push your blood pressure too low.

If that happened, you might feel dizzy, lightheaded, flushed, faint, or just unusually wiped out, which can overlap with your PAH symptoms. To be fair, this is only a theoretical concern based on lab and animal work, not strong human proof. Still, since iloprost already drops blood pressure, please check with your pharmacist or doctor before adding Danshen.

Aurlumyn (iloprost injection, given intravenously) is approved to treat severe frostbite in adults, specifically to reduce the risk of losing fingers or toes (digit amputations). Its effectiveness was established in young, healthy adults who experienced frostbite at high altitudes.
