Why this applies to Carbamazepine

Carbamazepine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the situation with Danshen and your carbamazepine. Your body relies on a liver and gut enzyme called CYP3A4 to break carbamazepine down. In a small study of healthy men, taking Danshen for about 10 to 14 days revved up that enzyme, meaning it could clear carbamazepine out of your system faster. If your levels drop too low, your seizures (or facial nerve pain or mood symptoms) may not stay well controlled.

Interestingly, a single dose of Danshen seemed to do the opposite and briefly raise drug levels, which could mean more dizziness, drowsiness, or unsteadiness. Because carbamazepine has a narrow safe range, even modest shifts matter. Please don't add or stop Danshen without checking with your doctor or pharmacist first.

What Is Carbamazepine Used For?

Brand names: Tegretol, Tegretol XR, Equetro, Carbatrol, Carnexiv

Carbamazepine is used to treat certain types of seizures — including partial seizures, generalized tonic-clonic (grand mal) seizures, and mixed seizure patterns. It works best for those specific seizure types; it does not appear to control absence (petit mal) seizures.

It is also prescribed to relieve the intense facial pain of trigeminal neuralgia — a nerve condition that causes sudden, severe pain along the face. It is not a general painkiller and shouldn't be used for ordinary aches and pains.
