Betrixaban falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Danshen interacts with.

Theoretically, danshen may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Danshen has been reported to have antithrombotic effects. Animal research also suggests that taking a danshen combination formula with clopidogrel exhibits a synergistic increase in antiplatelet aggregation and prolongation of coagulation time when compared with either taken alone.

Here's the thing to keep in mind: Betrixaban (Bevyxxa) is already a blood thinner, given to keep dangerous clots from forming while you're in the hospital. Danshen, on its own, seems to have its own blood-thinning, anti-clotting effects. So when you pair the two, you're essentially stacking two things that discourage clotting, and that can tip you toward more bleeding than intended.

What might that look like? Easy bruising, nosebleeds, bleeding gums, blood in the urine or stool, or cuts that won't stop. To be fair, this concern comes mostly from animal studies and case reports, not strong human trials, so it's more of a theoretical caution. Still, since bleeding is serious, please don't combine these without checking with your pharmacist or doctor first.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the thing about Betrixaban: your body relies heavily on a transporter called P-glycoprotein to move this drug along and keep its levels in check. Danshen appears to slow that transporter down. In a small study, it pushed levels of another P-gp drug up by roughly a third. If the same happens with Betrixaban, more of it could stay in your system than expected.

Why that matters: Betrixaban is a blood thinner, so higher levels could mean a greater chance of bleeding, things like easy bruising, nosebleeds, bleeding gums, or blood in the urine or stool. On top of that, Danshen itself may affect clotting, which could add to the risk. This isn't a reason to panic, but please check with your pharmacist or doctor before combining them.
