Why this applies to Ivabradine

Ivabradine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing with ivabradine: your body clears it using a liver and gut enzyme called CYP3A4. Danshen can nudge that enzyme, and the direction depends on how you take it. Taken regularly for a couple of weeks, danshen seems to speed up CYP3A4, which could lower ivabradine levels and make it less effective. If that happens, you might notice your heart rate creeping back up and less benefit for your heart failure.

Interestingly, a single dose of danshen did the opposite in studies, raising drug levels, which could mean a slower heart rate, dizziness, or those harmless flashes of brightness in your vision ivabradine is known for. The evidence here is early, so don't panic, but please check with your pharmacist or doctor before combining them.

About Ivabradine

What Is Ivabradine Used For?

Brand names: Procoralan, Coralan, Corlentor, Corlanor

Ivabradine (including the brand Corlanor) is used in adults with stable, symptomatic chronic heart failure with reduced pumping function to reduce the risk of being hospitalized for worsening heart failure. It is prescribed when the heart is beating too fast at rest and the patient is either already on the highest tolerated dose of a beta-blocker or cannot take one.

Corlanor is also approved to treat stable, symptomatic heart failure caused by dilated cardiomyopathy (an enlarged, weakened heart muscle) in children ages 6 months and older who have an elevated heart rate.
