Danshen + Prednisolone
Prednisolone brand names: Prednisolone, Sterane, Delta Cortef, Cortalone, Pred Forte, and others.
Prednisolone falls into 2 drug categories that Danshen can interact with; the most serious is rated moderate. Danshen might alter the levels of drugs cleared by p-glycoprotein.

These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Danshen interacts with whole categories of medication. Prednisolone falls into 2 of those categories, so it can interact with Danshen in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
P-GLYCOPROTEIN SUBSTRATES
1 of 2Why this applies to Prednisolone
Prednisolone falls under P-GLYCOPROTEIN SUBSTRATES — the group Danshen interacts with.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: Danshen may slow down a cellular pump called P-glycoprotein, which helps shuttle certain drugs out of your body. Prednisolone is partly handled by that same pump, so adding Danshen could let a bit more of the steroid stick around than usual.
In practice, that might mean slightly stronger steroid effects, things like trouble sleeping, mood changes, higher blood sugar, fluid retention, or stomach upset. I want to be honest though: the proof here is modest. The main study used a different drug (fexofenadine) and showed only a moderate rise, so we're reasoning by category, not from solid prednisolone data. Culinary tastes likely won't matter, but concentrated Danshen supplements could. Before combining them, please check with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Prednisolone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the deal with pairing Danshen and prednisolone. Your body uses a liver and gut enzyme called CYP3A4 to break down prednisolone. Research suggests that taking Danshen steadily for a week or two can actually speed up that enzyme, which means your body could clear prednisolone faster than usual. In plain terms, your steroid might not work as strongly as it should.
That matters because prednisolone is often controlling something real, like inflammation, asthma, an autoimmune flare, or transplant protection. If it gets cleared too quickly, you could see those symptoms creep back. Interestingly, a single dose of Danshen may briefly do the opposite. The evidence here is only moderate, so don't panic, but please check with your pharmacist or doctor before combining them.

From our pharmacist’s overview on the Prednisolone page
“Prednisolone quiets inflammation and dampens the immune system, and its list of 202 interacting supplement and herbal ingredients, four of them rated major, deserves a careful read rather than a shrug. Grapefruit can push prednisolone levels up by blocking its breakdown, St.”

About Danshen
What is Danshen?
- Heart and circulation support
- Angina (chest pain)
- High blood pressure
- Anxiety and stress
- Liver health
Danshen is a root used in traditional Chinese medicine, mainly for heart and circulation problems. Some early research is promising, but the evidence is not strong enough to confirm it works for any condition, and it may affect blood clotting. Talk to a pharmacist or doctor before using it, especially if you take other medicines or have a health condition.
About Prednisolone
What Is Prednisolone Used For?
Brand names: Prednisolone, Sterane, Delta Cortef, Cortalone, Pred Forte, Solupred
Oral prednisolone (tablets, oral solutions, syrup and orally disintegrating tablets) is used to control inflammation or an overactive immune system. Depending on the product, this includes severe allergic conditions, skin diseases, hormone (endocrine) disorders, ulcerative colitis, Crohn's disease, certain blood disorders, and rheumatic (joint and muscle) conditions.
Prednisolone eye drops (Pred Forte, Pred Mild, prednisolone acetate and prednisolone sodium phosphate ophthalmic products) treat steroid-responsive inflammation of the front parts of the eye, including the eyelid, conjunctiva, cornea and sclera. Pred Mild is for mild to moderate noninfectious allergic and inflammatory eye disorders. Not every product is approved for every use.
