Danshen and Triamcinolone

Here is the interaction we found between these two, with the evidence behind it.

Moderate

P-GLYCOPROTEIN SUBSTRATES

Why this applies to Triamcinolone

Triamcinolone falls under P-GLYCOPROTEIN SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.
Severity Moderate Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: Danshen may slow down a pump in your body called p-glycoprotein, which helps move certain medicines out of your system. Triamcinolone is one of those medicines that this pump can carry. If the pump works more slowly, a bit more triamcinolone could stick around in your body than usual.

What might that look like in real life? Slightly stronger steroid effects, things like fluid retention, a higher blood sugar, mood changes, or trouble sleeping. To be fair, the evidence here is limited (it comes from a study using a different drug, fexofenadine), so this is more of a possible concern than a sure thing. Still, before pairing Danshen with your triamcinolone, please check with your pharmacist or doctor first.
