Acetaminophen, Chlorpheniramine, Dextromethorphan Hydrobromide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the simple version for this Coricidin combo. In test-tube studies, Danshen seems to slow down a liver enzyme called CYP1A2. If that happened in your body, drugs cleared by that enzyme could build up a bit higher than usual. For this product, that mainly points to the chlorpheniramine (the antihistamine) and dextromethorphan (the cough suppressant). In theory, you might feel a little more drowsy, dizzy, or foggy than expected.

The good news: this is only laboratory evidence, and when researchers tested a real CYP1A2 drug in people, Danshen did not actually change its levels. So a meaningful problem is unlikely. Still, since Danshen can also affect clotting, run this combination past your pharmacist before mixing them.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

This combo cold product (Coricidin HBP) includes dextromethorphan, the cough suppressant, which is partly handled by an enzyme called CYP3A4. Danshen can nudge this enzyme in either direction. Taken steadily for a week or two, it appears to speed up CYP3A4, which could lower dextromethorphan levels and make it a bit less effective for your cough. Oddly, a single dose of danshen seems to do the opposite and raise levels for a short time, which might bump up drowsiness or dizziness.

Honestly, this is based on limited early research using a different drug, so the real-world effect here is probably small. The acetaminophen and chlorpheniramine pieces are not the main concern. Still, if you take danshen, mention it to your pharmacist before pairing the two.

This combination product (Coricidin HBP) contains acetaminophen (Tylenol) for pain and fever, chlorpheniramine for allergy symptoms, and dextromethorphan for cough. The piece that matters here is the acetaminophen, since a liver enzyme called CYP2E1 helps process it. In lab and animal studies, certain compounds in Danshen appear to slow that enzyme down. In theory, that could let a little more acetaminophen linger in the body.

I want to be honest about how soft this evidence is. It comes only from test-tube and animal research, and it has never actually been reported in people. So this isn't a reason to panic if you've taken them together. Still, since acetaminophen can stress the liver in higher amounts, it's worth a quick chat with your pharmacist or doctor before pairing them, especially if you use other products that also contain acetaminophen.

GLUCURONIDATED DRUGS

Theoretically, danshen might affect the levels and clinical effects of drugs requiring glucuronidation.
In vitro research shows that danshen induces the expression of glucuronosyltransferases. However, it also inhibits the activity of glucuronosyltransferases, including various members of the 1A and 2B families. The extent of inhibition of a specific glucuronosyltransferase seems to be dependent on whether or not the danshen is processed via 'sweating'. This type of processing may affect the levels of constituents in danshen that alter glucuronosyltransferase activity. So far, this interaction has not been reported in humans.

This combo cold-and-flu product (Coricidin HBP) contains acetaminophen for aches and fever, chlorpheniramine for runny nose and sneezing, and dextromethorphan for cough. Acetaminophen in particular is partly cleared by a process called glucuronidation, where your liver attaches a sugar-like tag to help wash the drug out. Danshen has shown mixed effects on those glucuronidation enzymes in the lab, sometimes speeding them up and sometimes slowing them down, so the net direction in a real person is genuinely uncertain.

Here is the honest part: this is only a theoretical, test-tube concern and has never been reported in actual people. If danshen did slow clearance, acetaminophen could linger a bit longer, but at normal doses that is unlikely to cause trouble. Just be sure you are not doubling up on acetaminophen from other products, and check with your pharmacist before combining them.
