P-GLYCOPROTEIN SUBSTRATES

Prednisone falls under P-GLYCOPROTEIN SUBSTRATES — the group Danshen interacts with.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: your body uses a little pump called P-glycoprotein to move certain medicines out of cells, and Danshen may slow that pump down a bit. In one study, Danshen raised levels of a different P-glycoprotein drug by roughly a third. Prednisone interacts with this same pump, so combining them could nudge your prednisone levels a little higher than usual.
If that happens, you might notice more of prednisone's usual side effects, things like trouble sleeping, mood changes, a bigger appetite, stomach upset, or fluid retention. This is rated moderate and the evidence is only fair, so it isn't a sure thing. Please don't stop or change anything on your own. Just check with your pharmacist or doctor before adding Danshen.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the deal with combining Danshen and prednisone. Your liver and gut use an enzyme called CYP3A4 to help process prednisone. In one small human study, taking Danshen for about 10 to 14 days actually sped up that enzyme, which could mean your body clears prednisone a bit faster than usual. In practice, that might make prednisone work a little less well for whatever it's treating, whether that's inflammation, a flare-up of arthritis, asthma, or an autoimmune condition.
I'll be honest, the evidence here is limited and based on a different drug (midazolam), so this is more of a heads-up than a red alert. Interestingly, a single dose of Danshen seemed to do the opposite. Before you mix the two, please check with your pharmacist or doctor.

Prednisone is used as an anti-inflammatory or immunosuppressive medicine (one that lowers immune activity). It treats many conditions across several body systems.
