Danshen and Betamethasone Dipropionate

Here is the interaction we found between these two, with the evidence behind it.

Moderate

P-GLYCOPROTEIN SUBSTRATES

Why this applies to Betamethasone Dipropionate

Betamethasone Dipropionate falls under P-GLYCOPROTEIN SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the good news for this pairing: the Danshen interaction on file deals with drugs that travel through the body using a transporter called p-glycoprotein. But betamethasone dipropionate (Sernivo) is a cream or ointment you rub onto your skin, not a pill you swallow. Very little of it gets into your bloodstream, so the whole-body transporter effect that worried us with oral drugs really doesn't come into play here.

The research behind this concern used a swallowed drug, so it doesn't map cleanly onto a topical steroid. I wouldn't lose sleep over using Danshen while treating your eczema or psoriasis patches. That said, Danshen can thin the blood a bit, so if you take other medicines or have a bleeding issue, run it past your pharmacist or doctor first.

From our pharmacist’s overview on the Betamethasone Dipropionate page

“Betamethasone dipropionate (Sernivo) is a potent corticosteroid applied to the skin, and enough can absorb through it, especially over large areas or with long courses, that these interactions deserve genuine vigilance.
