Pazopanib falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Danshen interacts with.

Theoretically, danshen may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Danshen has been reported to have antithrombotic effects. Animal research also suggests that taking a danshen combination formula with clopidogrel exhibits a synergistic increase in antiplatelet aggregation and prolongation of coagulation time when compared with either taken alone.

Here's something worth knowing if you take Votrient (pazopanib) for kidney cancer. This medicine already carries a bleeding risk on its own, since drugs that block tumor blood vessel growth can make bleeding, bruising, and nosebleeds more likely. Danshen appears to have its own blood-thinning, anti-clotting activity, so stacking the two together could add up and raise your chance of bleeding problems.

I'll be honest about the evidence: this is mostly theoretical and based on animal studies and case reports, not solid human trials with pazopanib specifically. Still, because bleeding with this cancer drug can be serious, I'd play it safe. Please don't start or stop anything on your own. Talk with your oncologist or pharmacist first so they can keep an eye on you.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist for Votrient (pazopanib): your body partly relies on a transporter called P-glycoprotein, sort of a cellular bouncer, to move this medicine along and help clear it. Danshen appears to slow that bouncer down. In one study with a different P-gp drug, Danshen pushed blood levels up by about a third. So pairing Danshen with pazopanib could nudge your pazopanib levels higher than expected.

Why that matters: more pazopanib can mean more of its usual side effects, like high blood pressure, diarrhea, fatigue, nausea, or changes in liver tests. Danshen may also thin the blood a bit, and pazopanib already carries a bleeding risk. The evidence here is limited, so don't panic, but please run this by your oncologist or pharmacist before adding Danshen.

Pazopanib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the situation with Votrient (pazopanib) and Danshen. Pazopanib is an active cancer drug (not a prodrug), and your body clears it using a liver and gut enzyme called CYP3A4. The research we have suggests that taking Danshen for a week or two can speed that enzyme up, which would clear pazopanib out faster and lower its levels. For a kidney cancer treatment, weaker drug levels could mean it doesn't work as well as it should.

There's a twist: a single dose of Danshen seems to do the opposite and raise levels, which could increase side effects like high blood pressure, fatigue, diarrhea, or liver strain. Because the evidence comes from small studies (not from pazopanib itself), it's not certain. Please don't start or stop anything on your own. Loop in your oncologist or pharmacist first.

What Is Pazopanib Used For?

Brand name: Votrient

Pazopanib tablets — sold as the brand Votrient and as generic pazopanib — are used to treat two types of advanced cancer in adults: advanced renal cell carcinoma (RCC), the most common form of kidney cancer, and advanced soft tissue sarcoma (STS) in patients who have already received prior chemotherapy.

Important limitation: Pazopanib has not been shown to work in adipocytic soft tissue sarcoma (a subtype involving fat cells) or in gastrointestinal stromal tumors (GISTs), so it is not used for those cancers.
