Rivaroxaban falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Danshen interacts with.

Theoretically, danshen may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Danshen has been reported to have antithrombotic effects. Animal research also suggests that taking a danshen combination formula with clopidogrel exhibits a synergistic increase in antiplatelet aggregation and prolongation of coagulation time when compared with either taken alone.

Here's the simple version: Xarelto (rivaroxaban) is a blood thinner that lowers your blood's ability to form clots. Danshen, on its own, also seems to thin the blood and slow clotting. Put the two together and you may be stacking two effects that push in the same direction, which can raise your risk of bleeding.

With rivaroxaban, that could show up as easy bruising, nosebleeds, bleeding gums, blood in your urine or stool, or cuts that take longer to stop. I'll be honest about the proof here: this concern is mostly theoretical and based on animal studies and case reports, not solid human trials. Still, because bleeding is serious, it's worth a quick chat with your pharmacist or doctor before mixing them. Please don't start or stop anything on your own.

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the concern with pairing Danshen and Xarelto (rivaroxaban). Your body partly relies on a transporter called p-glycoprotein to move rivaroxaban along and help clear it. Danshen appears to slow that transporter down. In a small study, it raised levels of another p-glycoprotein drug by about a third. So in theory, Danshen could let more rivaroxaban build up than usual.

With a blood thinner, that's worth taking seriously, since higher levels can mean a greater risk of bruising and bleeding, like nosebleeds, bleeding gums, or blood in the urine or stool. On top of that, Danshen itself may affect clotting, which could add to that risk. The evidence here isn't strong, but please don't combine them without checking with your pharmacist or doctor first.

Rivaroxaban also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing to know about pairing Danshen with Xarelto (rivaroxaban). Your body clears rivaroxaban partly through an enzyme called CYP3A4, and Danshen can nudge this enzyme in either direction. When taken regularly for a week or two, Danshen seems to speed that enzyme up, which could lower rivaroxaban levels and make it work less well. That matters because rivaroxaban is a blood thinner, and weaker blood thinning could mean a higher risk of the very clots it's meant to prevent. A single dose, oddly, may do the opposite and raise levels, which leans toward more bruising or bleeding.

On top of that, Danshen itself may affect clotting, so the bleeding concern is real. This is moderate-strength evidence, not certain, but please loop in your pharmacist or doctor before combining them.

About Rivaroxaban

What Is Rivaroxaban Used For?

Brand name: Xarelto

Rivaroxaban tablets are used to reduce the risk of stroke in adults with nonvalvular atrial fibrillation (an irregular heart rhythm), treat deep vein thrombosis (DVT, a blood clot in a vein) and pulmonary embolism (PE, a blood clot in the lungs), and lower the chances of these clots coming back. They also help prevent clots after hip or knee replacement surgery, protect hospitalized patients at risk for clots, and — in combination with aspirin — reduce the risk of major cardiovascular events in adults with coronary artery disease (CAD) or peripheral artery disease (PAD).

Rivaroxaban tablets are also approved for treating VTE (venous thromboembolism, blood clots in veins) and preventing recurrence in pediatric patients from birth to under 18 years, and for preventing clots in children aged 2 and older with congenital heart disease after the Fontan procedure. Rivaroxaban for oral suspension (available as Rivaroxaban Granule and Xarelto oral suspension) is specifically indicated for these two pediatric uses.
