Why this applies to Nifedipine

Nifedipine falls under ANTIHYPERTENSIVE DRUGS — the group Danshen interacts with.

Theoretically, taking danshen with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.

Here's the gist: nifedipine already relaxes your blood vessels to bring blood pressure down. Danshen, in animal studies, seems to do something similar and may gently lower blood pressure on its own. So if you pair the two, the worry is that their effects could stack up and push your blood pressure a little too low.

If that happened, you might feel dizzy, lightheaded, tired, or notice your heart racing, especially when you stand up quickly. I'll be honest with you though, this concern comes mostly from lab and animal research, so it's more theoretical than proven in people. Still, since both can affect blood pressure, it's worth a quick chat with your pharmacist or doctor before combining them, and keep an eye on your readings.

Nifedipine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the deal with Danshen and your nifedipine. Your body relies on an enzyme called CYP3A4 to break nifedipine down, and Danshen can mess with how fast that enzyme works. In one small study, taking Danshen for about 10 to 14 days actually sped up this enzyme, which could lower your nifedipine levels and make it work less well. That might mean your blood pressure creeps back up or your chest pain isn't as well controlled.

Oddly, a single dose seemed to do the opposite and could raise levels, which might cause more dizziness, flushing, headache, or swollen ankles. The evidence is limited, so I wouldn't panic. Just don't start Danshen without checking with your pharmacist or doctor first.

What Is Nifedipine Used For?

Brand names: Procardia XL, Adalat PA, Adalat, Nifedin, Nifed PA, Nifed, Procardia, Adalat XL, Adalat CC, Adalat LA, Adalat Retard, Adipine MR, Adipine XL, Cardilate MR, Coracten SR, Coracten XL, Fortipine LA 40, Hypolar Retard 20, Nifedipress MR, Nifopress Retard, Tensipine MR

Nifedipine treats high blood pressure (hypertension), chronic stable angina (chest pain brought on by physical effort), and vasospastic angina (chest pain caused by spasm of the coronary arteries, sometimes called Prinzmetal's angina). Lowering blood pressure with nifedipine helps reduce the risk of strokes and heart attacks.

The capsule form is approved for vasospastic angina and chronic stable angina only — it is not approved for high blood pressure. The extended-release tablet (including Procardia) covers all three uses: vasospastic angina, chronic stable angina, and high blood pressure.
