Apixaban falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Danshen interacts with.

Theoretically, danshen may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Danshen has been reported to have antithrombotic effects. Animal research also suggests that taking a danshen combination formula with clopidogrel exhibits a synergistic increase in antiplatelet aggregation and prolongation of coagulation time when compared with either taken alone.

Here's the simple version: Apixaban (Eliquis) is already a blood thinner, and Danshen has its own blood-thinning, anti-clotting effects. When you stack the two together, those effects can add up. That means a higher chance of bleeding, which with apixaban can show up as easy bruising, nosebleeds, bleeding gums, pink or red urine, dark stools, or bleeding that takes longer than usual to stop.

I'll be honest about the evidence here. This warning is mostly theoretical and based on anecdotal reports and animal studies, not solid human trials. Still, because bleeding is serious, I wouldn't combine them on your own. Please check with your pharmacist or doctor before adding Danshen, and watch closely for any unusual bleeding if you do.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing with Eliquis (apixaban) and danshen. Your body clears apixaban partly through an enzyme called CYP3A4, and danshen can tug on that enzyme in two directions. Taken steadily for a week or two, danshen seems to speed that enzyme up, which could lower apixaban levels and weaken its clot-prevention, meaning a higher risk of the very strokes or clots it's meant to stop. A single dose, oddly, may do the opposite and raise levels, which could mean more bruising or bleeding.

On top of that, danshen itself may thin the blood, so the bleeding worry stacks up. The evidence here is early and modest, but apixaban is too important to gamble with. Please check with your pharmacist or doctor before combining them.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the thing to know if you take Eliquis (apixaban) and are thinking about Danshen. Your body partly relies on a pump called P-glycoprotein to move apixaban out of your system. Danshen appears to slow that pump down. In one study in healthy people, it raised levels of another drug handled by this same pump. So Danshen could let a bit more apixaban build up in your blood than expected.

Why does that matter? Apixaban is a blood thinner, so higher levels can mean a greater chance of bruising or bleeding, things like nosebleeds, bleeding gums, or blood in the urine or stool. On top of that, Danshen itself may thin the blood, which adds to the risk. The evidence here isn't rock solid, but I'd play it safe. Please check with your pharmacist or doctor before combining them.

About Apixaban

What Is Apixaban Used For?

Brand name: Eliquis

Apixaban tablets (sold as Eliquis and under the generic name apixaban) are used to reduce the risk of stroke and systemic embolism (clots traveling to other parts of the body) in adults with nonvalvular atrial fibrillation (an irregular heart rhythm not caused by a heart valve problem). They are also used to prevent deep vein thrombosis (DVT — a blood clot in the leg) after hip or knee replacement surgery, and to treat DVT and pulmonary embolism (PE — a blood clot in the lungs), as well as to lower the risk of those clots coming back.

Eliquis is additionally approved to treat venous thromboembolism (VTE — blood clots in the veins) and reduce the risk of recurrent VTE in pediatric patients from birth and older, following at least 5 days of initial anticoagulant treatment.
