Alteplase, tPA falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Danshen interacts with.

Theoretically, danshen may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Danshen has been reported to have antithrombotic effects. Animal research also suggests that taking a danshen combination formula with clopidogrel exhibits a synergistic increase in antiplatelet aggregation and prolongation of coagulation time when compared with either taken alone.

Here's the key thing to know: Alteplase (Activase) is a powerful clot-busting drug given in the hospital for emergencies like a heart attack, stroke, or major blood clot in the lungs. It works by actively dissolving clots, so bleeding is already its main risk. Danshen, on the other hand, seems to thin the blood and slow clotting on its own.

Stacking the two together could, in theory, push bleeding risk even higher, things like easy bruising, bleeding that won't stop, or more serious internal bleeding. To be fair, this concern is mostly theoretical and based on animal and anecdotal reports, not strong human studies. Still, since alteplase is used in critical situations, it's smart to tell your care team about any Danshen use so they can keep you safe.
