Danshen has been reported to have antithrombotic effects. Animal research also suggests that taking a danshen combination formula with clopidogrel exhibits a synergistic increase in antiplatelet aggregation and prolongation of coagulation time when compared with either taken alone.

Here's the concern with pairing Danshen and warfarin. Warfarin already thins your blood by lowering your clotting ability, and Danshen seems to push in the same direction by reducing how easily your blood clots. Stack the two and you may get more blood thinning than intended, which can show up as easy bruising, nosebleeds, bleeding gums, blood in your urine or stool, or cuts that won't stop bleeding.

Warfarin is a narrow therapeutic index drug, meaning small shifts can tip you toward bleeding or clotting. The evidence here is mostly theoretical and anecdotal, not strong human studies, but because bleeding is serious I'd treat this combo cautiously. Please don't start, stop, or adjust anything on your own. Talk with your pharmacist or doctor first, and watch your INR closely if you do use both.

In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the thing with warfarin and Danshen. Warfarin is a true blood thinner, not a prodrug, so anything that slows its breakdown can let it build up in your system. The lab data we have suggests Danshen can block CYP1A2, one of the liver enzymes that helps clear warfarin. In theory, that could push your blood toward being too thin, raising your risk of bruising, nosebleeds, bleeding gums, or more serious bleeding.

Now, I'll be honest with you: this particular concern comes from test-tube studies, and it didn't show up in people when another CYP1A2 drug was tested. Still, warfarin has a very narrow safety window, so even small shifts matter. Please don't start Danshen without checking with your doctor or pharmacist first, and keep up with your INR testing.

Here's the gist with warfarin and Danshen. Warfarin is cleared from your body partly by a liver enzyme called CYP2C9. In lab studies, compounds in Danshen appear to slow that enzyme down. If that happened in a real person, warfarin could build up and thin your blood more than intended. With a finicky drug like warfarin, where the dose is fine-tuned, even a small shift can matter. Signs to watch for would be easy bruising, nosebleeds, bleeding gums, or blood in urine or stool.

I'll be honest: this is mostly theoretical and based only on test-tube data, not real patients. Danshen may also affect clotting on its own. Please don't start or stop anything without checking with your doctor or pharmacist first, since your INR may need monitoring.

Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing about warfarin: it's a tricky blood thinner where even small shifts in its level can tip you toward bleeding or, on the flip side, leave you under-protected against clots. The interaction on file involves the CYP3A4 enzyme. Based on early research, taking Danshen for a week or two may speed that enzyme up, which could lower warfarin's effect and theoretically let clots form. A single dose, oddly, may do the opposite.

But there's a bigger concern with this pair. Danshen itself appears to thin the blood, so combined with warfarin you could see more bleeding risk: easy bruising, nosebleeds, bleeding gums, or blood in the urine. Please don't start or stop anything on your own. Check with your pharmacist or doctor first, since your INR may need watching.

Theoretically, danshen may increase the risk of bleeding if used with warfarin.
There have been several case reports of increased international normalized ratio (INR) after concomitant use of danshen and warfarin. Elevations in INR have occurred as early as 3-5 days after start of danshen. However, a clinical trial in adults taking warfarin with stable INR found that the addition of compound danshen dripping pills, containing danshen extract, Panax notoginseng, and borneol, 270 mg three times daily for 4 weeks did not alter INR levels or the average required warfarin dose when compared to baseline. These findings are consistent with animal research, which found no change in warfarin pharmacokinetics with the use of danshen. Other research in healthy adult males also shows that taking a combination of danshen and kudzu with warfarin does not increases plasma warfarin area under the curve, but may reduce plasma soluble thrombomodulin levels. However, other research shows that danshen might increase the rate of absorption and decrease the elimination rate of warfarin. Also, research in healthy adult males shows that taking a combination of danshen and kudzu with warfarin increases plasma area under the curve of danshensu, a constituent of danshen, by approximately 29.5-fold. Danshen should be used cautiously in patients taking warfarin.

Here's the deal with combining Danshen and warfarin: warfarin already keeps your blood from clotting too easily, and Danshen may add to that effect. There have been real case reports of people's INR (the blood test that tells us how thin your blood is) jumping higher after starting Danshen, sometimes within just 3 to 5 days. A higher INR means a greater chance of bleeding problems, like easy bruising, nosebleeds, bleeding gums, or in serious cases blood in the urine or stool.

Now, the evidence is mixed. One trial found no change at all, so this isn't a guaranteed problem. But warfarin is a narrow-margin medicine where small shifts matter, so I'd never just add Danshen on my own. Please check with your pharmacist or doctor first, and if you do start it, more frequent INR checks are a smart move.

What Is Warfarin Used For?

Brand names: Coumadin, Panwarfin, Sofarin

Warfarin sodium tablets and Jantoven tablets are both used to prevent and treat venous thrombosis (blood clots in the veins) and pulmonary embolism (a clot in the lungs). They are also used to prevent clot-related complications in people with atrial fibrillation (an irregular heart rhythm) or who have had a heart valve replaced.

In people who have had a heart attack, warfarin is used to reduce the risk of death, another heart attack, stroke, or other clotting events. It does not dissolve an existing clot, but it helps prevent the clot from growing and stops new dangerous clots from forming.
