Why this applies to Apomorphine

Apomorphine falls under P-GLYCOPROTEIN SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist for Apokyn (apomorphine). Danshen seems to slow down a transporter called p-glycoprotein, which helps move certain drugs out of your body. In one small human study, danshen pushed the blood levels of a p-glycoprotein drug up by roughly 30 to 37 percent. So if apomorphine is handled the same way, danshen could let a bit more of it linger in your system.

What might that feel like? Possibly more of apomorphine's usual effects, such as strong nausea, dizziness, sudden drops in blood pressure, sleepiness, or more involuntary movements. This is a moderate, possible interaction based on limited evidence, so don't panic, but please run it by your pharmacist or neurologist before combining them.

Apomorphine also falls under GLUCURONIDATED DRUGS.

Theoretically, danshen might affect the levels and clinical effects of drugs requiring glucuronidation.
In vitro research shows that danshen induces the expression of glucuronosyltransferases. However, it also inhibits the activity of glucuronosyltransferases, including various members of the 1A and 2B families. The extent of inhibition of a specific glucuronosyltransferase seems to be dependent on whether or not the danshen is processed via 'sweating'. This type of processing may affect the levels of constituents in danshen that alter glucuronosyltransferase activity. So far, this interaction has not been reported in humans.

Here's the short version on combining Danshen with your apomorphine (Apokyn). Apomorphine is cleared from your body partly through a process called glucuronidation, and in lab and animal studies Danshen can nudge those clearing enzymes in both directions, sometimes speeding them up and sometimes slowing them down. So we can't even say cleanly which way it would push apomorphine levels.

The important thing: this has never actually been reported in people, and it's based only on test-tube and animal work. If anything did happen, slowed clearance could mean more apomorphine side effects like nausea, dizziness, sleepiness, low blood pressure, or fainting. I wouldn't lose sleep over it, but please run the Danshen by your pharmacist or doctor before starting it, especially since it can also thin the blood.

Apomorphine Injection treats “off” episodes in advanced Parkinson's disease. These are periods of poor movement and stiffness, sometimes called hypomobility. They include “end-of-dose wearing off” and unpredictable “on/off” episodes.

It is used for acute, intermittent treatment, meaning as-needed treatment of episodes as they happen. It has been studied as an add-on to other Parkinson's medicines.
