Theoretically, danshen may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Danshen has been reported to have antithrombotic effects. Animal research also suggests that taking a danshen combination formula with clopidogrel exhibits a synergistic increase in antiplatelet aggregation and prolongation of coagulation time when compared with either taken alone.

Here's the simple version: dabigatran (Pradaxa) is already a powerful blood thinner that keeps clots from forming. Danshen, on its own, also appears to slow down clotting and reduce how easily your blood platelets stick together. So when you take the two together, you're essentially stacking two things that thin the blood, and the worry is added bleeding.

For dabigatran, that could mean easy bruising, nosebleeds, bleeding gums, blood in the urine or stool, or, less often, a more serious bleed. I'll be honest about the evidence: this is mostly theoretical and based on animal studies, not solid human trials. Still, because Pradaxa is potent, I wouldn't combine them on your own. Please check with your pharmacist or doctor first.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the thing with Pradaxa (dabigatran): your body relies on a little transporter called p-glycoprotein to move it out of your system. Danshen appears to slow that transporter down. In one study, danshen pushed levels of another p-glycoprotein drug up by about 30 to 37 percent. The same effect could let dabigatran build up a bit higher than expected.

Why does that matter? Dabigatran is a blood thinner, and on top of that, danshen itself may affect clotting. So the real worry is extra bleeding: easy bruising, nosebleeds, bleeding gums, or more serious bleeding. This isn't a reason to panic, but please check with your pharmacist or doctor before combining them rather than stopping anything on your own.

About Dabigatran

What Is Dabigatran Used For?

Brand names: Pradaxa, Pradax

Dabigatran etexilate capsules are approved for adults to reduce the risk of stroke and systemic embolism (blood clots traveling through the body) in non-valvular atrial fibrillation (an irregular heart rhythm not caused by a heart valve problem). Capsules are also used to treat deep vein thrombosis (DVT — a blood clot in a deep vein, usually the leg) and pulmonary embolism (PE — a clot in the lungs), to prevent those clots from coming back, and to prevent DVT and PE in adults who have had hip replacement surgery.

For children, the capsule form is approved for treating VTE (venous thromboembolism — blood clots in veins) and reducing recurrence in patients aged 8 to under 18. Pradaxa Oral Pellets are approved specifically for children aged 3 months to under 12 years for the same treatment and recurrence-prevention purposes.
