Aspirin, Caffeine, Codeine Phosphate falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Danshen interacts with.

Theoretically, danshen may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Danshen has been reported to have antithrombotic effects. Animal research also suggests that taking a danshen combination formula with clopidogrel exhibits a synergistic increase in antiplatelet aggregation and prolongation of coagulation time when compared with either taken alone.

This combination product contains aspirin, which is a blood thinner that keeps your platelets from sticking together. Danshen is thought to thin the blood too. When you pair them, the two can add up and raise your chance of bleeding or bruising more easily. Watch for things like nosebleeds that won't stop, bleeding gums, dark or bloody stools, or bruises that show up without a bump.

To be fair, this is mostly based on lab and animal work plus a few case reports, so it's more of a theoretical concern than a proven one. Still, since aspirin already affects clotting, I'd play it safe. Please check with your pharmacist or doctor before adding Danshen, especially before any surgery or dental work.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

In this combination product, the part that interacts with Danshen through CYP1A2 is mostly the caffeine. Caffeine is broken down by that same liver enzyme, so if Danshen slows it down, caffeine could build up a bit. That might leave you feeling jittery, anxious, or with a racing heart and trouble sleeping. The good news is this is only a theoretical concern based on lab studies. When researchers actually tested a similar CYP1A2 drug in real people, they did not see this effect.

Separately, keep in mind Danshen may thin the blood, and this product already contains aspirin, so together they could raise your bruising or bleeding risk. That is worth a quick chat with your pharmacist or doctor before combining them.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing with this combo pill: the codeine part needs your body to turn it into morphine before it relieves pain, so it acts like a prodrug. Danshen mostly affects CYP3A4, which is only a side route for codeine, so the bigger concern is different. Danshen can thin the blood, and your tablet already contains aspirin, which also affects clotting. Together they could raise your risk of bruising, stomach upset, or bleeding.

The CYP3A4 evidence here is early and mixed (taking Danshen for a while may speed clearance, while a single dose may slow it), so I wouldn't lose sleep over the codeine piece. Just check with your pharmacist or doctor before pairing them, especially because of the aspirin.

Theoretically, danshen may increase the levels of aspirin and the risk of bleeding.
Research in healthy adult males shows that taking a combination of danshen and kudzu with aspirin increases plasma aspirin area under the curve by approximately 3.4-fold. Animal research also shows that taking a combination of danshen and kudzu (danshen-gegen formula) with aspirin increases maximal blood levels of aspirin and salicylic acid by approximately 4-fold and 3.7-fold, respectively, without impacting blood loss. Taking danshen increases the antiplatelet activity of aspirin and might increase the side effects of aspirin.

This combination pill contains aspirin, and that is the piece Danshen interacts with. Danshen on its own can thin the blood a little, and it may also raise aspirin levels in your body. In one study in healthy men, pairing Danshen (with kudzu) and aspirin pushed aspirin exposure to roughly three times higher. More aspirin in your system, plus Danshen's own blood-thinning effect, can stack up.

For you, that mostly means a higher chance of bleeding and bruising, things like easy bruising, nosebleeds, stomach irritation, or dark stools. The codeine and caffeine parts are not the concern here. This is partly theoretical and partly from real studies, so it is not a reason to panic, but please check with your pharmacist or doctor before combining them.
