The piece to focus on here is the caffeine inside this combination product. Both caffeine and ephedra are stimulants, so taking them together can stack their effects on your heart and nervous system. That can mean a racing or pounding heartbeat, jitteriness, trouble sleeping, and most importantly a bump in blood pressure. For some people that added strain on the cardiovascular system can be genuinely dangerous.

To be honest, this is the lesser of your worries with ephedra. It is banned from supplements in the U.S. precisely because it has been linked to strokes and serious heart events, and the codeine and aspirin pieces don't make that any safer. The evidence for this specific pairing is mostly anecdotal, but the concern is real. Please talk with your pharmacist or doctor about safer options instead.

The piece that matters here is the caffeine in this combination product. Caffeine is a stimulant, and ephedra (ma huang) is a much stronger one. Stacking two stimulants together doesn't just add them up, it can really pile on the effects. That can mean a racing or pounding heart, jitteriness, trouble sleeping, a spike in blood pressure, and in serious cases things like an abnormal heart rhythm, seizures, or even a heart attack or stroke.

This warning comes mostly from real-world reports rather than tightly controlled studies, but the harms linked to ephedra are serious enough that I wouldn't take any chances. Ephedra is also banned in U.S. supplements for exactly these reasons. Please loop in your pharmacist or doctor before combining these, and ask about safer options.

The piece of this combination product I want to focus on is the caffeine. Some animal studies hint that ephedra may speed up an enzyme called CYP1A2, which is the main pathway your body uses to clear caffeine. If that happens, caffeine could be broken down a little faster than usual. Honestly, the bigger worry here runs the other way: both ephedra and caffeine are stimulants, so stacking them can pile on jitteriness, a racing or pounding heart, anxiety, trouble sleeping, and higher blood pressure.

Keep in mind this enzyme effect is theoretical and based only on lab and animal data, so it's far from certain. Ephedra itself is banned in U.S. supplements for good reason. Please talk with your pharmacist or doctor before mixing these.
