Doxepin falls under QT INTERVAL-PROLONGING DRUGS — the group Ephedra interacts with.

Theoretically, ephedra might have an additive effect with drugs that prolong the QT interval.
Even in healthy volunteers, EKG changes including prolonged QT interval and premature atrial contractions have been reported with ingestion of recommended doses of ephedra. Ephedra may have an additive effect with drugs that prolong the QT interval. This may increase the risk of ventricular arrhythmias.

Here's the concern with pairing ephedra and doxepin. Doxepin is a tricyclic antidepressant, and like others in its family, it can stretch out a part of your heart's electrical cycle called the QT interval. Ephedra's stimulants (ephedrine and pseudoephedrine) can do the same thing, even at recommended doses. Stack the two together and the effects add up, which can raise the risk of a dangerous, irregular heartbeat.

This isn't doxepin building up in your body, it's two things tugging on your heart rhythm in the same direction. Symptoms to watch for would be a racing or pounding heartbeat, dizziness, or fainting. The evidence here is on the weaker side and partly theoretical, but the potential outcome is serious. Keep in mind ephedra is banned in U.S. supplements anyway. Please talk with your pharmacist or doctor before combining anything like this.
