Why this applies to Nortriptyline
Nortriptyline falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ephedra interacts with.
Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for nortriptyline (Pamelor). Your body partly clears this antidepressant through a liver enzyme called CYP1A2. The concern on file is that ephedra may speed up that enzyme, which in theory could push nortriptyline out of your system faster and leave less of it working. That might mean your depression isn't controlled as well as it should be.
Honestly though, this is based only on animal lab data, so it's theoretical and not well proven. The bigger worry is ephedra itself: it's a banned stimulant, and combining a stimulant with a tricyclic like nortriptyline can strain your heart and raise blood pressure. Please talk with your pharmacist or doctor before mixing these, and ask about safer options.
