Nortriptyline falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Grapefruit interacts with.

Theoretically, grapefruit juice might increase levels of drugs metabolized by CYP1A2.
In vitro research suggests that grapefruit juice might inhibit CYP1A2 enzymes. So far, this interaction has not been reported in humans.

Here's the gist with grapefruit and your nortriptyline. There's lab-based evidence that grapefruit may slow down a liver enzyme called CYP1A2, which is one of the enzymes that helps clear nortriptyline from your body. If that enzyme is blocked, the drug could build up a bit and you might feel its side effects more strongly: things like drowsiness, dry mouth, constipation, blurry vision, dizziness when standing, or a faster heartbeat.

That said, this one is mostly theoretical. It comes from test-tube studies and hasn't actually been reported in real people, so I wouldn't lose sleep over it. Still, since this medication takes some careful dosing to get right, it's worth mentioning to your pharmacist or doctor before you make grapefruit a daily habit.
