Why this applies to Omacetaxine Mepesuccinate

Omacetaxine Mepesuccinate falls under P-GLYCOPROTEIN SUBSTRATES — the group Grapefruit interacts with.

Grapefruit juice does not seem to affect renal P-glycoprotein (P-gp). Theoretically, it might inhibit intestinal P-gp, but evidence is conflicting.
While most in vitro research shows that grapefruit products inhibit P-gp,, research in humans is less clear. Two small clinical studies in healthy adults using digoxin as a probe substrate show that grapefruit juice does not inhibit P-gp in the kidneys. It is unclear whether this applies to intestinal P-gp, for which digoxin is not considered to be a sensitive probe. Grapefruit juice has been shown to reduce levels of fexofenadine, and increase levels of quinidine. However, as both of these drugs are also substrates of other enzymes and transporters, it is unclear what role, if any, intestinal P-gp has in these findings.

Here's the good news first: omacetaxine (Synribo) is given as an injection under the skin, not as a pill you swallow. The main worry with grapefruit and this kind of drug is that grapefruit might block a transporter called P-glycoprotein in your gut, which could change how much medicine your body absorbs. Since omacetaxine skips your gut entirely, that gut-level effect really doesn't come into play here.

On top of that, the evidence for grapefruit affecting P-glycoprotein in people is shaky and conflicting, and this interaction is rated as minor. So a glass of grapefruit juice is very unlikely to change how omacetaxine works or worsen side effects like low blood counts, fatigue, or nausea. Still, because you're being treated for CML, it's always smart to run your diet and supplements by your oncology pharmacist or doctor.

What Is Omacetaxine Injection Used For?

  • Omacetaxine injection is used to treat adults with chronic myelogenous leukemia (CML; a type of cancer of the white blood cells) who have already been treated with at least two other medications for CML and can no longer benefit from these medications or cannot take these medications due to side effects. Omacetaxine injection is in a class of medications called protein synthesis inhibitors. It works by slowing the growth of cancer cells.
