Why this applies to Alvimopan

Alvimopan falls under P-GLYCOPROTEIN SUBSTRATES — the group Grapefruit interacts with.

Grapefruit juice does not seem to affect renal P-glycoprotein (P-gp). Theoretically, it might inhibit intestinal P-gp, but evidence is conflicting.
While most in vitro research shows that grapefruit products inhibit P-gp,, research in humans is less clear. Two small clinical studies in healthy adults using digoxin as a probe substrate show that grapefruit juice does not inhibit P-gp in the kidneys. It is unclear whether this applies to intestinal P-gp, for which digoxin is not considered to be a sensitive probe. Grapefruit juice has been shown to reduce levels of fexofenadine, and increase levels of quinidine. However, as both of these drugs are also substrates of other enzymes and transporters, it is unclear what role, if any, intestinal P-gp has in these findings.

Here's the short version on grapefruit and Entereg (alvimopan). Alvimopan is carried by a transporter in your gut called P-glycoprotein, and grapefruit is sometimes said to block that transporter. In theory, blocking it could let a bit more alvimopan get absorbed. But honestly, the human evidence here is weak and conflicting. Studies in people haven't clearly shown grapefruit changes how this transporter works in the gut, so any real-world effect on alvimopan is uncertain.

On top of that, alvimopan is usually given only for a short time in the hospital after bowel surgery, so regularly drinking grapefruit juice during that window isn't a typical scenario. Still, it's smart to mention any grapefruit habit to your surgical team or pharmacist so they can keep an eye on things.

What Is Alvimopan Used For?

Brand name: Entereg

Alvimopan capsules are used in hospitals to help the digestive tract recover more quickly after surgery that involves removing part of the bowel (partial bowel resection with primary anastomosis — meaning the remaining ends are reconnected). After this type of surgery, the bowel often temporarily stops working, a condition called postoperative ileus.

This medication is only given during a short hospital stay and is not for long-term or home use.
