CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Dextromethorphan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Grapefruit interacts with.
Clinical research shows that grapefruit juice can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. When taken orally, effects of grapefruit juice on CYP3A4 levels appear to last at least 48 hours. Grapefruit's ability to inhibit CYP3A4 has even been harnessed to intentionally increase levels of venetoclax, which is metabolized by CYP3A4, in an elderly patient with acute myeloid leukemia who could not afford full dose venetoclax. The lower dose of venetoclax in combination with grapefruit juice resulted in serum levels of venetoclax in the therapeutic reference range of full dose venetoclax and positive treatment outcomes for the patient.
Professional consensus recommends the consideration of patient age, existing medical conditions, additional medications, and the potential for additive adverse effects when evaluating the risks of concomitant use of grapefruit juice with any medication metabolized by CYP3A4. While all patients are at risk for interactions with grapefruit juice consumption, patients older than 70 years of age and those taking multiple medications are at the greatest risk for a serious or fatal interaction with grapefruit juice.
Well-controlled human studies have demonstrated the likely existence of this interaction.
Here's the gist for grapefruit and dextromethorphan (the cough suppressant in products like Benylin and Delsym). Grapefruit blocks a liver enzyme called CYP3A4, which is one of the pathways your body uses to clear this medicine. When that pathway slows down, more dextromethorphan can build up in your system, and that means a bigger effect than you bargained for.
In real life, that could mean feeling more drowsy, dizzy, lightheaded, or even a bit confused or restless. The good news is that an occasional cough syrup taken with breakfast isn't usually a crisis for most healthy adults. But if you drink grapefruit juice regularly, are over 70, or take several other medicines, it's worth a quick chat with your pharmacist or doctor before mixing the two.

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the gist for cough syrups with dextromethorphan: your body normally clears this medicine using a liver enzyme called CYP3A4. Grapefruit puts the brakes on that enzyme, so the drug can hang around longer and build up to higher levels than expected. This isn't a prodrug situation, so more drug in your system means a stronger effect.
In real life that could mean feeling more drowsy, dizzy, lightheaded, or a bit nauseated or foggy. A study (not a randomized trial, but solid enough) backs this up. Concentrated grapefruit juice is more of a concern than an occasional grapefruit segment. Since this comes up with regular grapefruit intake, just run it by your pharmacist or doctor before mixing the two.

Dextromethorphan temporarily relieves cough due to minor throat and bronchial irritation. This can happen with the common cold or from inhaled irritants.
Some products are also labeled to help you get to sleep by reducing the urge to cough. Some labels also list helping reduce how intense the coughing is. Honey Works syrup, Premier Value Tussin Cough and CVS Long Acting Cough liquids are also labeled to help loosen phlegm (mucus) and thin bronchial secretions so they drain from the bronchial tubes.
