QT INTERVAL-PROLONGING DRUGS
1 of 2Why this applies to Trimipramine
Trimipramine falls under QT INTERVAL-PROLONGING DRUGS — the group Grapefruit interacts with.
Clinical research in healthy volunteers shows that drinking 6 liters of grapefruit juice over 6 hours prolonged the QTc by a peak amount of 14 milliseconds (ms). This prolongation was similar to the QT prolongation caused by the drug moxifloxacin. In individuals with long QT syndrome, a smaller dose of grapefruit juice, 1.5 liters, resulted in a greater peak QTc prolongation of about 30 ms. The effect of smaller quantities of grapefruit juice on the QT interval is unclear.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's something many people don't realize about grapefruit: in larger amounts it can stretch out your heart's electrical cycle slightly, what we call the QT interval. Trimipramine, like other tricyclic antidepressants, can also lengthen the QT interval on its own. Stack the two together and the effects can add up, which in rare cases raises the risk of an irregular heartbeat.
The good news is that an occasional half grapefruit or a small glass of juice with breakfast is unlikely to cause trouble. The studies showing real QT changes used very large amounts (think liters of juice). I'd still mention your grapefruit habit to your pharmacist or doctor, especially if you have a known heart rhythm problem or take other medicines that affect the QT interval, so they can decide what's safe for you.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES
2 of 2Trimipramine also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.
In vitro research suggests that grapefruit juice might inhibit CYP2C9 enzymes. So far, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with grapefruit and your trimipramine. The concern is that grapefruit may slow down an enzyme in your liver called CYP2C9, which helps break this antidepressant down. If that pathway slows, a bit more trimipramine could linger in your system. Since this is an active drug (not a prodrug), that would mean a slightly stronger effect, and possibly more of the usual tricyclic side effects like drowsiness, dry mouth, constipation, dizziness, or a faster heartbeat.
The reassuring part: this is only a theoretical, test-tube finding. It has not actually been seen in people, so I wouldn't panic. Still, if you enjoy grapefruit regularly, it's worth a quick chat with your pharmacist or doctor to be safe.

Trimipramine capsules are indicated for the relief of symptoms of depression. Endogenous depression — depression that arises from within, rather than from an obvious outside cause — is more likely to respond to treatment than other types.
In clinical studies, trimipramine worked as well as imipramine in hospitalized patients with depression, and was comparable to amitriptyline in less severely depressed outpatients.
